Cognitive Behavioral Therapy has become the dominant form of psychotherapy in North America. The CBT model is theoretically based on the idea that all external and internal stimuli are filtered through meaning-making, consciously accessible cognitive schemas. The goal of CBT is to identify dysfunctional or maladaptive thoughts and beliefs, and replace them with more adaptive cognitive interpretations. While CBT is clearly effective as a treatment, there is good reason to be skeptical that its efficacy is due to the causal mechanisms (...) posited by the CBT model. This paper will argue that the specific cognitive schemas posited by the CBT model likely do not play a direct role in the development or treatment of psychological illness. Cognitive schemas, as identified in CBT interventions, are likely to be the result of patient confabulation and epistemically under-supported practitioner-based identification. CBT interventions appear to impose coherence on patients’ psychological states, rather than identifying and modifying preexistent causally efficacious core beliefs. (shrink)

While classifications of mental disorders have existed for over one hundred years, it still remains unspecified what terms such as 'mental disorder', 'disease' and 'illness' might actually denote. While ontologies have been called in aid to address this shortfall since the GALEN project of the early 1990s, most attempts thus far have sought to provide a formal description of the structure of some pre-existing terminology or classification, rather than of the corresponding structures and processes on the side of the patient. (...) We here present a view of mental disease that is based on ontological realism and which follows the principles embodied in Basic Formal Ontology and in the application of BFO in the Ontology of General Medical Science. We analyzed statements about what counts as a mental disease provided in the research agenda for the DSM-V, and in Pies' model. The results were used to assess whether the representational units of BFO and OGMS were adequate as foundations for a formal representation of the entities in reality that these statements attempt to describe. We then analyzed the representational units specific to mental disease and provided corresponding definitions. Our key contributions lie in the identification of confusions and conflations in the existing terminology of mental disease and in providing what we believe is a framework for the sort of clear and unambiguous reference to entities on the side of the patient that is needed in order to avoid these confusions in the future. This essay is divided into two parts, deeply intermingled. Part I examines not only the origin of conscious experience but also how it is possible to ask of our own consciousness how it came to be. Part II examines the origin of experience itself, which soon reveals itself as the ontological question of Being. The chief premise of Part I is that symbolic communion and the categorizations of language have enabled human organisms to distinguish between themselves as actually existing entities (...) and their own immediate experience of themselves and their world. This enables them to reflect upon abstract concepts, including “self,” “experience,” and “world.” Symbolic communication and conceptualization grow out of identification, the act of first observing conscious experiencing and intimating what it is like, mimesis, a gestural protolanguage learned through imitation, and reflection, seeing oneself through the eyes of others. The step into actual intentional speech is made through self-assertion, narrative, and intersubjectivity. These three become the spiral of human cultural development that includes not only the adaptive satisfaction of our biological needs, but also the creativity of thought. With the mental-conceptual separation of subject and object – of self and world – the human ability to witness the universe (and each other) is the ground of our genuinely human quality. Consciousness gives human life its distinctively human reality. It is, therefore, one and the same ability that enables us to shape planet Earth by means of conceptual representations (rather than by means of our hands alone) while also awakening us to the significance of being. -/- Looking beyond human self-consciousness to investigate the origin and nature of awareness itself in Part 2, reductive objective materialism is found to be of little use. Direct experience also falls short in that, in order to be transformed into objective knowledge about itself, it must always be interpreted through and limited by the symbolic contexts of culture and the idiosyncratic conceptualizations of the individual. Awareness-in-itself must thus be considered ultimately unexplainable, but this may more indicate its inexpressible transcendence of all symbolic qualifiers than its nonexistence. It is suggested that awareness is not “self-aware” (as in deity) but is instead unknowing yet identical with the only true universal: the impetus of creative unfolding. Our human knowledge, as an expression of this unfolding, is seen to emerge from our conscious experiencing and, in turn, to have the power – and enormous responsibility – of directing that experience. Our underlying symbolic worldviews are found to be autopoietic: They limit or open our conscious experience, which, in turn, confirms those worldview expectations. As we explore a future of unforeseeable technological breakthroughs on an ailing planet who patiently copes with our “success,” truly vital decisions about the nature, meaning, extent, and future of conscious experience will have to be made. The rapid increase of the old age people imposes the reconsideration of the rehabilitation techniques and procedures and/or the development of the existing ones, at least from two points of view: the limitation use of the pharmaceutical drugs because of their secondary effects in the debilitated organisms and their avoidance; the high risk of the induced anxiety states, depression or other symptoms as a consequence of the main disease, i.e. the neuro-degenerative or mobility dysfunctions, limiting again the use of such (...) excessive drug medication. Moreover, Alzheimer’s or Parknson’s diseases are otherwise incurable, so the application of rehabilitating procedures like music-based therapy becomes preferential. The advances both in the identification of the specific auditory musical-sensitive circuits and of the non- musical neuro-connections but activated indirectly by music, otherwise inaccessible by other techniques, and in the understanding of consciousness, which can be described by seven cognitive centers, allows to improve and develop the music based therapy. A multitask procedure based on the activation of these cognition centers is presented, allowing to increase the personal psychical engagement of the patient in the recommended music based therapeutic program, addressed both to the rehabilitation of the main degenerative disease and to the secondary induced dysfunctions. This procedure combines the evidence based medicine assisted by laboratory expertise for the determination of a correct diagnostic, and narrative based medicine allowing to quantify the personal engaging qualities of the patient with respect to the proposed music based program, reducing in this way the gap between these two basic procedures and bringing in a first plane the patient care, not only the disease, with beneficial consequence for the therapy efficiency and for the patient-medical care relations. This study has proven that the economic system is determined by various components, in particular, it includes the real sector of the economy, which is formed on mega-, macro, meso-, micro-and nano-levels. In addition, it was proved that the nano-level is determined by the activities of individuals whose economic activity begins with the birth and attitude of parents, attending various educational and upbringing institutions, and studying at university. A separate segment of the nano-level of the economic system is the baby (...) industry responsible for the production of goods and services for children and future parents. All these aspects are indicators of the development of the baby economy. Such an economic category as the baby economy was separated and defined. The study uses the following methods: analysis, synthesis, structural analysis, systemic approach, observation, comparison, multifactor regression. These methods made it possible to achieve the results, which involve isolation of the baby economy as a component of nanoeconomy and definition of such components of the baby economic segment of nanoeconomy as a family or household economy, the baby industry, the economy of the educational process and upbringing. For this purpose, the value-institutional approach was applied. In addition, multifactorial analysis of the impact of indicators of the baby economy development on the population of a country with a transitive economy with incomes below the subsistence minimum was performed. This analysis identified a direct but minor relationship between these phenomena, which demonstrates the need to intensify and create a policy for the baby economy in similar states. The theoretical significance of the obtained results is determined by the introduction of a new economic category of "the baby economy" for the formation of a nano economic component within socio-economic systems. Journal bearings play an important role on many rotating machines placed on industrial environments, especially in steam turbines of thermoelectric power plants. Babbitt damage (BD) and excessive clearance (C) are usual faults of steam turbine journal bearings. This paper is focused on achieving an effective identification of these faults through an intelligent recognition approach. The work was carried out through the processing of real data obtained from an industrial environment. In this work, a feature selection procedure was applied in (...) order to choose the features more suitable to identify the faults. This feature selection procedure was performed through the computation of typical testors, which allows working with both quantitative and qualitative features. The classification tasks were carried out by using Nearest Neighbors, Voting Algorithm, Naïve Associative Classifier and Assisted Classification for Imbalance Data techniques. Several performance measures were computed and used in order to assess the classification effectiveness. The achieved results (e.g., six performance measures were above 0.998) showed the convenience of applying pattern recognition techniques to the automatic identification of BD and C. (shrink)

It has been noticed that self-referential, ambiguous definitional formulas are accompanied by complementary self-referential antinomy formulas, which gives rise to contradictions. This made it possible to re-examine ancient antinomies and Cantor’s Diagonal Argument (CDA), as well as the method of nested intervals, which is the basis for evaluating the existence of uncountable sets. Using Georg Cantor’s remark that every real number can be represented as an infinite digital expansion (usually decimal or binary), a simplified system for verifying the definitions of (...) real numbers, subsets, and strings was created - the Cantor Criterion, which allows faulty definitions to be pointed out. For the CDA, the connection of formulas defining objects (real numbers, subsets, strings) from outside the list with supplementary formulas was demonstrated - their indirect and indispensable nature testifies to the lack of unambiguity and gives rise to contradictions for Cantor’s antinomic formulas. Thus, Cantor’s theorem about the higher power of the set of all subsets, using the reductio ad absurdum proof, lost its power and it was indicated that it is necessary to correct the scheme of the Axiom of Specification, which was introduced precisely to exclude antinomies from set theory by excluding from the use of self-referential antinomies and ambiguous supplementary formulas coupled with them identified by the H hypothesis. The method of nested intervals was investigated and it was shown that every real number can be defined by the limit of nested intervals and a countable list of real numbers obtained from a countable pool of all Jules Richard’s texts. Agricultural productivity is something on which Indian economy highly depends. This is the one of the reasons that disease detection in plants plays a vital role in agriculture field, as having disease in plants are unavoidable. If proper care is not taken in this area, then it causes serious effects on plants and due to which the overall agriculture yield will be affected. For instance, a disease named little leaf disease is a hazardous disease found in pine trees in United (...) States. Detection of plant disease through some automatic technique is beneficial as it reduces a large work of monitoring in big farms of crops, and at very early stage itself if detected properly by identifying the symptoms of diseases can result in increased productivity. This paper presents an algorithm for image segmentation technique which is used for automatic detection and classification of plant leaf diseases. It also covers diseases classification techniques that can be used for plant leaf disease detection. Image segmentation is one of the method which will segment the raw images in to two or more clusters and the programmed algorithm will work fine in analyzing these clusters for disease classification and prediction of type of disease that a plant leaf gets affected. Purpose. To determine whether highlights are rapidly identified at early levels of vision. -/- Methods. Visual search experiments were carried out using simple black and white figures corresponding to shiny objects lit from various directions. These included, for example, depictions of cylinders with highlights positioned at various heights (see figure). Targets and distractors differed only in the arrangement of their constituent regions, allowing them to be distinguished by the position of the highlights on the corresponding objects. -/- Results. Three observers (...) were tested. Search is rapid when the target corresponds to a shiny black object with a bottom highlight among objects with top highlights (view figure from rightside up). However, search slows down when the target corresponds to an object with a top highlight among objects with bottom highlights (view figure from upside down). This asymmetry in search speeds is destroyed when the contrast of the figures is reversed. -/- Conclusions. These results indicate that regions in the image can be rapidly identified as highlights at early levels of vision. They also suggest--as for several other rapid recovery processes--that an assumption of lighting from above is used as a simplifying constraint. (shrink)

Aim: To elicit preferences for prognostic information, attitudes towards withdrawal of life-sustaining treatment (WLST) and perspectives on acceptable quality of life after post-anoxic coma within the adult general population of Germany, Italy, the Netherlands and the United States of America. Methods: A web-based survey, consisting of questions on respondent characteristics, perspectives on quality of life, communication of prognostic information, and withdrawal of life-sustaining treatment, was taken by adult respondents recruited from four countries. Statistical analysis included descriptive analysis and chi2-tests for (...) dierences between countries. Results: In total, 2012 respondents completed the survey. In each country, at least 84% indicated they would prefer to receive early prognostic information. If a poor outcome was predicted with some uncertainty, 37-54% of the respondents indicated that WLST was not to be allowed. A conscious state with severe physical and cognitive impairments was perceived as acceptable quality of life by 17–44% of the respondents. Clear differences between countries exist, including respondents from the U.S. being more likely to allow WLST than respondents from Germany (OR = 1.99, p < 0.001) or the Netherlands (OR = 1.74, p < 0.001) and preferring to stay alive in a conscious state with severe physical and cognitive impairments more than respondents from Italy (OR = 3.76, p < 0.001), Germany (OR = 2.21, p < 0.001), or the Netherlands (OR = 2.39, p < 0.001). Conclusions: Over one-third of the respondents considered WLST unacceptable when there is any remaining prognostic uncertainty. Respondents had a more positive perspective on acceptable quality of life after coma than what is currently considered acceptable in medical literature. This indicates a need for a closer look at the practice of WLST based on prognostic information, to ensure responsible use of novel prognostic tests. (shrink)

Discussions of patient-centred care and patient autonomy in bioethics have tended to focus on the decision-making context and the process of obtaining informed consent, leaving open the question of how patients ought to be counselled in the daily maintenance of their health and management of chronic disease. Patient activation is an increasingly prominent counselling approach and measurement tool that aims to improve patients’ confidence and skills in managing their own health conditions. The strategy, which has received little conceptual (...) or ethical analysis, raises important questions about how clinicians ought to foster confidence and a sense of control in their patients without exposing them to blame, stigma and other harms. In this paper, we describe patient activation, discuss its relationship to personal responsibility, autonomy and health disparities, and make recommendations regarding its use and measurement. Starting with the results of Li et al. in 1992 there is valuable interest in finding long range correlations in DNA sequences since it raises questions about the role of introns and intron-containing genes. In the present paper we studied two sequences that are the human T-cell receptor alpha/delta locus, Gen-Bank name HUMTCRADCV, a noncoding chromosomal fragment of M = 97630 bases (composed of less than 10% of coding regions), and the Escherichia Coli K12, Gen-Bank name ECO110K, a genomic fragment (...) with M = 111401 bases consisting of mostly coding regions and containing more that 80% of coding regions. We attributed the value (+1) to the purines and the value (-1) to the pirimidines and to such reconstructed random walk we applied the method of the Recurrence Quantification Analysis(RQA) that was introduced by Zbilut and Webber in 1994. By using dimension D=1 and Embedded Dimensions D=3 and D=5, we obtain some indicative results. Also by a simple eye examination of the reconstructed maps, the differences between coding and non coding regions are evident and impressive and consist in the presence in noncoding regions of long patches of the same colour that are absent in the coding sequence. At first sight this suggests a simple explanation to the concept of „long-range‟ correlation. On the quantitative plane, we used the %Rec., the %Det., the Ratio, the Entropy, the %Lam., and the Lmax that, as explained in detail in the text, represent the basic variables of RQA. The significant result that we have here is that both Lmax and Laminarity exhibit very large values in HUMTCRADCV and actually different in values respect to ECO110K where such variables assume more modest values. Therefore we suggest that there is the observed difference between HUMTCRADCV and ECO110K. The claimed higher long-range correlations of introns respect to exons from many authors may be explained here in reasonof such found higher values of Lmax and of Laminarity in HUMTCRADCV respect to ECO110K. The article at hand is an attempt to clarify some misunderstandings and non-logical conclusions that some scholars have arrived to in regards to the authorship and chronology found in the book of Daniel. Also, it appears that there is not a clear understanding of the roles played by Cyrus and Darius as co-regents of the Medopersian empire. In addition, many scholars attribute the second part of Daniel (chapters 7-12) to a pseudo Daniel from the 2nd century b.C. For millennia people have speculated about the nature of time—without success. Time plays a role with all processes and events studied by all the disciplines. It is reported here that the existence of time is a direct consequence of the existence of space. Space exists, and it continues to exist. Space is there, and it continues to be there. Space exists as place, the three-dimensional place that matter can occupy. The three-dimensional extension of spatial-place is measured with a ruler of (...) some kind. Place, three-dimensionality, and extension are intrinsic-qualities of space itself. The continuing-existence of space is a direct consequence of the existence of space. The continuing-existence of space is measured with a clock. Spatial continuing-existence also has intrinsic-qualities. A list of the qualities of the continuing-existence of space appears to be the same as a list of the qualities of time. Seventeen qualities of the continuing-existence of space are compared to the equivalent qualities of time. There is an exact match between the qualities of time and the qualities of spatial continuing-existence. The qualities and roles of the continuing-existence of space fulfill all the known qualities and roles of time. Realizing that spatial continuing-existence is time makes time understandable, and clarifies the relations of time to matter, change, and process. (shrink)

In a rigorous systematic review, Dukhanin and colleagues categorize metrics and evaluative tools of the engagement of patient, public, consumer, and community in decision-making in healthcare institutions and systems. The review itself is ably done and the categorizations lead to a useful understanding of the necessary elements of engagement, and a suite of measures relevant to implementing engagement in systems. Nevertheless, the question remains whether the engagement of patient representatives in institutional or systemic deliberations will lead to improved clinical outcomes (...) or increased engagement of individual patients themselves in care. Attention to the conceptual foundations of patient engagement would help make this systematic review relevant to the clinical care of patients. (shrink)

Patients with serious illnesses or injuries may decide to quit their medical treatment if they think paying the fees will put their families into destitution. Without treatment, it is likely that fatal outcomes will soon follow. We call this phenomenon “near-suicide”. This study attempted to explore this phenomenon by examining how the seriousness of the patient’s illness or injury and the subjective evaluation of the patient’s and family’s financial situation after paying treatment fees affect the final decision on the (...) treatment process. Bayesian Mindsponge Framework (BMF) analytics were employed to analyze a dataset of 1042 Vietnamese patients. We found that the more serious the illnesses or injuries of patients were, the more likely they were to choose to quit treatment if they perceived that paying the treatment fees heavily affected their families’ financial status. Particularly, only one in four patients with the most serious health issues who thought that continuing the treatment would push themselves and their families into destitution would decide to continue the treatment. Considering the information-filtering mechanism using subjective cost–benefit judgments, these patients likely chose the financial well-being and future of their family members over their individual suffering and inevitable death. Our study also demonstrates that mindsponge-based reasoning and BMF analytics can be effective in designing and processing health data for studying extreme psychosocial phenomena. Moreover, we suggest that policymakers implement and adjust their policies (e.g., health insurance) following scientific evidence to mitigate patients’ likelihood of making “near-suicide” decisions and improve social equality in the healthcare system. Patients are generally assumed to have the right to choices about treatment, including the right to refuse treatment, which is constrained by considerations of cost‐effectiveness. Independently, many people support the idea that patients who are responsible for their ill health should incur penalties that non‐responsible patients do not face. Surprisingly, these two areas have not received much joint attention. This paper considers whether restricting the scope of responsibility to pre‐treatment decisions can be justified, or whether a demand (...) to hold people responsible for 'usual suspect' choices such as smoking or failure to exercise commits us to also holding people responsible for their treatment choices. I argue that there is no good reason to support this restriction: those who advocate responsibility for (some) pre‐treatment choices should also advocate responsibility for (some) treatment choices. However, I also note that, as with pre‐treatment choices, patients may sometimes have reason to choose in ways that do not optimize their health. As such, I also consider a process, based on the idea of public reasons, for deciding which treatment choices patients cannot legitimately be held responsible for, along with a method for considering proposed changes to this category. Particularly, only one in four patients with the most serious health issues who thought that continuing the treatment would push themselves and their families into destitution would decide to continue the treatment. Considering the information-filtering mechanism using subjective cost–benefit judgments, these patients likely chose the financial well-being and future of their family members over their individual suffering and inevitable death. Our study also demonstrates that mindsponge-based reasoning and BMF analytics can be effective in designing and processing health data for studying extreme psychosocial phenomena. Moreover, we suggest that policymakers implement and adjust their policies (e.g., health insurance) following scientific evidence to mitigate patients’ likelihood of making “near-suicide” decisions and improve social equality in the healthcare system. Subjective rights enjoy limited import in Kelsenian theory for whereas the concept of duty underlies every legal norm, that of rights is merely possible and only emerges when the imposition of the sanction attached to the breach of the duty is made dependent upon a subject's will to bring legal action. The presence of secondary norms establishing certain duties of medical professionals on informed consent displays the existence of correlative reflex rights of patients. Yet, together with secondary norms, Western (...) legal systems typically institute norms of competence enabling patients to take legal action when those duties correlative to their reflex right to informed consent have been infringed. Thus, informed consent constitutes a set of subjective legal positions of patients and medical professionals. Despite its excessive formalism and questionable neutrality, the Kelsenian approach permits to define and clearly distinguish patients' genuine rights from mere aspirations devoid of legal basis. An accurate classification of bacteria is essential for the proper identification of patient infections and subsequent treatment decisions. Multi-Locus Sequence Typing (MLST) is a genetic technique for bacterial classification. MLST classifications are used to cluster bacteria into clonal complexes. Importantly, clonal complexes can serve as a biological species concept for bacteria, facilitating an otherwise difficult taxonomic classification. In this paper, we argue for the inclusion of terms relating to clonal complexes in biomedical ontologies.

Arguments are given against the thesis that properties and propositional functions are identical. The first shows that the familiar extensional treatment of propositional functions -- that, for all x, if f(x) = g(x), then f = g -- must be abandoned. Second, given the usual assumptions of propositional-function semantics, various propositional functions (e.g., constant functions) are shown not to be properties. Third, novel examples are given to show that, if properties were identified with propositional functions, crucial fine-grained intensional distinctions would (...) be lost. (shrink)

In this paper we propose that our understanding of pathocentric epistemic injustices can be enriched if they are theorised in terms of predicaments. These are the wider socially scaffolded structures of epistemic challenges, dangers, needs, and threats experienced by ill persons due to their particular emplacement within material, social, and epistemic structures. In previous work we have described certain aspects of these predicaments - pathocentric epistemic injustices, pathophobia, and so on. We argue that thinking predicamentally helps us integrate the various (...) social, epistemic, and practical obstacles experienced by ill persons and also unify the many concepts available for theorising them (microaggessions, epistemic injustices, and so on). A major part of philosophical work is engagement with argumentative texts. Engaging with an argumentative text involves correctly identifying the arguments presented in this text. In the context of teaching philosophy in school, the difficulty of correctly identifying arguments in philosophical texts is often underestimated. In this paper, I focus on one specific problem with argument identification that has been neglected in philosophy didactics thus far: the problem that there are many non-argumentative phenomena in an argumentative text that are (...) easily misidentified as expressing (part of) an argument. It is easy to fall for such phenomena if one is not aware of this problem, thereby misinterpreting the author’s intentions. —One of the promising applications of wireless sensor networks (WSNs) is monitoring of the human body for health concerns. For this purpose, a large number of small sensors are implanted in the human body. These sensors altogether provide a network of wireless sensors (WBANs) and monitor the vital signs and signals of the human body; these sensors will then send this information to the doctor. The most important application of the WBAN is the implementation of the monitoring network for patient (...) safety in the hospital environment. In this case, supporting patients’ mobility is one of the basic needs, which has been underestimated in recent studies. The problem that involves providing the required energy for the units used in this type of network is challenging; for this reason, sent/ received units with very low power consumption and with a very small radius are used in order to save energy. The resulting small sending range, leads to the lack of support for patients' mobility. In this paper, the AD HOC mode is suggested for use to establish a network and a multi-path routing algorithm, for the purpose of importing patients’ mobility in hospital setting. The results of the simulation show that in addition to supporting patients' mobility, the use of the proposed idea instead of previously presented protocols, reduces delays in data transmission and energy consumption; and it also increases the delivery rate depending on the destination and the lifetime of the network, while on the other hand, it increases routing overhead. S. Matthew Liao and Christian Barry argue that the patient-centered approach to deontology that I have developed—the restricting claims principle —‘is beset with problems.’ They think that it cannot correctly handle cases in which a potential victim sits in the path of an agent doing what she needs to do for some greater good, or in which a person’s property is used to benefit others and harm her. They argue that cases in which an agent does what would be permissible (...) but acts on a malicious reason show that agent intentions, rather than patientclaims, are fundamental to deontology. And they claim that the RCP presupposes the means principle in a way that shows that it is not really offering anything new. I argue here that all of these charges are mistaken. Doing so allows me to offer important refinements to the RCP, to highlight two common mistakes in reasoning about cases, and to set challenges for agentcentered approaches to deontology. (shrink)

The promotion of patient welfare is a central goal of medical professionals and serves as a fundamental principle of medical professionalism. Despite its importance, however, it is unclear what is meant by patient welfare. In my dissertation, I explore patient welfare by analyzing the ordinary notion of welfare and applying this analysis to the patient population. I argue that welfare is best understood and expressed in terms of capabilities, which I define using the structure and vocabulary of Basic Formal Ontology. (...) I conclude that patient welfare is a multi-dimensional quality of the human organism corresponding to the degree to which that patient has realized the array of capabilities typical of the human kind. Epistemic two-dimensional semantics, advocated by Chalmers and Jackson, among others, aims to restore the link between necessity and a priority seemingly broken by Kripke, by showing how armchair access to semantic intensions provides a basis for knowledge of necessary a posteriori truths. The most compelling objections to E2D are that, for one or other reason, the requisite intensions are not accessible from the armchair. As we substantiate here, existing versions of E2D are indeed subject to such access-based objections. But, we (...) moreover argue, the difficulty lies not with E2D but with the typically presupposed conceiving-based epistemology of intensions. Freed from that epistemology, and given the right alternative—one where inference to the best explanation provides the operative guide to intensions—E2D can meet access-based objections, and fulfill its promise of restoring the desirable link between necessity and a priority. This result serves as a central application of Biggs and Wilson, according to which abduction is an a priori mode of inference. (shrink)

The article analyzes new perspectives on the personality and professionalism of graduates of higher military educational institutions (hereinafter - HMEI), their motivation to military service in positions for the chosen military specialty. The bases of personality development of future military personnel and formation of clear professional self-identification are theoretically generalized. -/- It is noted that future officers, while undergoing training, must acquire the necessary knowledge and skills of their future profession, achieve their own personal, social, and professional efficiency and (...) success. It is during the period of studying at the HMEI that young men are at the stage of shaping and rooting of their self-consciousness, both personal and professional. -/- It is stressed that the military student must define for himself a military identity, as an awareness of the personality of his affiliation in the matter of protecting the freedom and independence of his state, the adoption of social norms, requirements and patterns of behavior, values and life style of military men. -/- It has been determined that at the HMEI the experience of identity of military students is actualized simultaneously in the professional field of civilian specialization and the military-professional sphere. -/- Professional self-identification of military students of HMEI is analyzed as a multidimensional dynamic non-linear process which determines the mechanisms, conditions, and results of structuring and re-structuring of pro­fessional identity of a person; the desire of the subject to master the knowledge, skills and self-diagnostic skills. Abstract: The Knowledge-Based System for Diagnosing Breast Cancer aims to support medical students in enhancing their education regarding diagnosis and counseling. The system facilitates the analysis of biopsy images under a microscope, determination of tumor type, selection of appropriate treatment methods, and identification of disease-related questions. According to the Ministry of Health's annual report in Gaza, there were 7,069 cases of breast cancer between 2009 and 2014, with 1,502 cases reported in 2014. In an era dominated by visual information, (...) where 65% of the population are visual learners, the Knowledge-Based System serves as an effective tool to aid long-term information retention by utilizing visual and textual resources. The system features a user-friendly interface, enabling students to diagnose the disease and expand their understanding. Furthermore, it can be accessed via smartphones. The Knowledge-Based System comprises three primary components: Firstly, it provides specific patient-oriented questions to familiarize students with the questioning process. Secondly, it facilitates the analysis of biopsy sample images to determine their integrity and identify potential infections. The previous Part I of the paper discusses the option of the Gödel incompleteness statement (1931: whether “Satz VI” or “Satz X”) to be an axiom due to the pair of the axiom of induction in arithmetic and the axiom of infinity in set theory after interpreting them as logical negations to each other. The present Part II considers the previous Gödel’s paper (1930) (and more precisely, the negation of “Satz VII”, or “the completeness theorem”) as a necessary condition for (...) granting the Gödel incompleteness statement to be a theorem just as the statement itself, to be an axiom. Then, the “completeness paper” can be interpreted as relevant to Hilbert mathematics, according to which mathematics and reality as well as arithmetic and set theory are rather entangled or complementary rather than mathematics to obey reality able only to create models of the latter. According to that, both papers (1930; 1931) can be seen as advocating Russell’s logicism or the intensional propositional logic versus both extensional arithmetic and set theory. Reconstructing history of philosophy, Aristotle’s logic and doctrine can be opposed to those of Plato or the pre-Socratic schools as establishing ontology or intensionality versus extensionality. Husserl’s phenomenology can be analogically realized including and particularly as philosophy of mathematics. One can identify propositional logic and set theory by virtue of Gödel’s completeness theorem (1930: “Satz VII”) and even both and arithmetic in the sense of the “compactness theorem” (1930: “Satz X”) therefore opposing the latter to the “incompleteness paper” (1931). An approach identifying homomorphically propositional logic and set theory as the same structure of Boolean algebra, and arithmetic as the “half” of it in a rigorous construction involving information and its unit of a bit. Propositional logic and set theory are correspondingly identified as the shared zero-order logic of the class of all first-order logics and the class at issue correspondingly. Then, quantum mechanics does not need any quantum logics, but only the relation of propositional logic, set theory, arithmetic, and information: rather a change of the attitude into more mathematical, philosophical, and speculative than physical, empirical and experimental. Hilbert’s epsilon calculus can be situated in the same framework of the relation of propositional logic and the class of all mathematical theories. The horizon of Part III investigating Hilbert mathematics (i.e. according to the Pythagorean viewpoint about the world as mathematical) versus Gödel mathematics (i.e. the usual understanding of mathematics as all mathematical models of the world external to it) is outlined. Ethical issues of information and communication technologies (ICTs) are important because they can have significant effects on human liberty, happiness, their ability to lead a good life. They are also of functional interest because they can determine whether technologies are used and whether their positive potential can unfold. For these reasons policy makers are interested in finding out what these issues are and how they can be addressed. The best way of creating ICT policy that is sensitive to ethical issues (...) would be to be proactive and address such issues at early stages of the technology life cycle. The present paper uses this position as a starting point and discusses how knowledge of ethical aspects of emerging ICTs can be gained. It develops a methodology that goes beyond established futures methodologies to cater for the difficult nature of ethical issues. The paper goes on to outline some of the preliminary findings of a European research project that has applied this method. (shrink)

We discuss the role of prior authorization (PA) in supporting patient-centered care (PCC) by directing health system resources and thus the ability to better meet the needs of individual patients. We begin with an account of PCC as a standard that should be aimed for in patient care. In order to achieve widespread PCC, appropriate resource management is essential in a healthcare system. This brings us to PA, and we present an idealized view of PA in order to argue (...) how at its best, it can contribute to the provision of PCC. PA is a means of cost saving and as such it has mixed success. The example of the US demonstrates how implementation of PA has increased health inequalities whereas best practice has the potential to reduce them. In contrast, systems of universal coverage, like those in Europe, may use the cost savings of PA to better address individuals' care and PCC. The conclusion we offer therefore is an optimistic one, pointing towards areas of supportive overlap between PCC and PA where usually the incongruities are most evident. (shrink)

It is aimed to evaluate of the effectiveness of the application of epidural steroid injection(ESE) in patients with lumbar disc herniation (LDH) in this study. Between November 2010- December 2011 patients applied Yahyalı State Hospital Algology Clinic withlow back pain for at least 3 months was evaluated in a prospective study. Application of the lumbar epidural steroid injection was planned for 150 patients. visual analogue scale (VAS) were used for scoring pain of patients. Besides, patients (...) with hand-finger floor distance and the degree of straight leg raising were evaluated before and after ESE injection. Ages of patients were between 27- 65 years old, of all patients 67 were men (44,7%), 83 were women (%55,3). Before and after treatment in VAS, direct leg increase, hand- finger ground distance of patients were found to be significantly different between measurements one by one (p=0,0037, p=0,0053, p=0,0062). Epidural steroid administration can provide earlier symptomatic treatment for the patients suffering from low back pain with lumbar disc herniation who did not respond to conservative treatment. Background: Despite the at least decades long record of philosophical recognition and interest, the intricacy of the deceptively familiar appearing concepts of ‘disease’, ‘disorder’, ‘disability’, etc., has only recently begun showing itself with clarity in the popular discourse wherein its newly emerging prominence stems from the liberties and restrictions contingent upon it. Whether a person is deemed to be afflicted by a disease or a disorder governs their ability to access health care, be it free at the point of use (...) or provided by an insurer; it also influences the treatment of individuals by the judicial system and employers; it even affects one’s own perception of self. -/- Aims: All existing philosophical definitions of disease struggle with coherency, causing much confusion and strife, and leading to inconsistencies in real-world practice. Hence, there is a real need for an alternative. -/- Materials & Methods: In the present article I analyse the variety of contemporary views of disease, showing them all to be inadequate and lacking in firm philosophical foundations, and failing to meet the desideratum of patient driven care. -/- Results: Illuminated by the insights emanating from the said analysis, I introduce a novel approach with firm ethical foundations, which foundations are rooted in sentience, that is the subjective experience of sentient beings. -/- Discussion: I argue that the notion of disease is at best superfluous, and likely even harmful in the provision of compassionate and patient-centred care. -/- Conclusion: Using a series of presently contentious cases illustrate the power of the proposed framework which is capable of providing actionable and humane solutions to problems that leave the current theories confounded. Patient RB has a peculiar memory impairment wherein he experiences his memories in rich contextual detail, but claims to not own them. His memories do not feel as if they happened to him. In this paper, I provide an explanatory model of RB’s phenomenology, the self-attentional model. I draw upon recent work in neuroscience on self-attentional processing and global workspace models of conscious recollection to show that RB has a self-attentional deficit that inhibits self-bias processes in broadcasting the contents of (...) episodic memories to the global workspace. Typically, self-related contents enjoy a higher salience level than other-related contents. Elimination of bias toward self-related contents diminishes the salience of those contents to the level of other-related contents. Because the typical high salience of self-related content is necessary for the feeling of ownership, RB lacks the feeling of ownership. I also discuss potential applications of the self-attentional model to other psychopathological cases. (shrink)

Cardiovascular diseases, including heart disease, pose a significant global health challenge, contributing to a substantial burden on healthcare systems and individuals. Early detection and accurate prediction of heart disease are crucial for timely intervention and improved patient outcomes. This research explores the potential of neural networks in predicting heart disease using a dataset collected from Kaggle, consisting of 1025 samples with 14 distinct features. The study's primary objective is to develop an effective neural network model for binary classification, identifying the (...) presence or absence of heart disease. The neural network architecture includes an input layer, a hidden layer, and an output layer, designed to capture intricate relationships within the dataset. Rigorous training and validation processes, accompanied by data preprocessing steps, ensure the model's robustness and generalization capabilities. The results demonstrate promising performance, with an accuracy of 92% and an average error of 0.062. Moreover, an analysis of feature importance highlights key predictors, including "oldpeak," "thalach," "trestbps," "ca," "thal," "cp," "chol," "sex," "restecg," "age," "slope," "fbs," and "exang." This research contributes to the field of predictive healthcare by leveraging neural networks to enhance heart disease prediction. The developed model offers the potential for early identification of individuals at risk, facilitating timely medical interventions and ultimately improving public health. Objective: This study aimed to investigate and compare the effects of tubular gastroesophageal anastomosis and traditional gastroesophageal supra-arch anastomosis on the postoperative lung function of patients with esophageal carcinoma. -/- Methods: A total of 90 patients with middle-lower segmental esophageal cancer admitted in our hospital from August 2014 to August 2016 were recruited to our study. They were randomly divided into two groups. The observation group underwent tubular gastroesophageal anastomosis, whereas the control group underwent traditional gastroesophageal supra-arch anastomosis. (...) Changes in the postoperative pulmonary function were compared between the two groups. -/- Results: The numbers of CD4 cells (27.45 ± 5.48), CD8 cells (35.97 ± 7.16), and natural killer cells (20.79 ± 2.49), as well as the CH4/CD8 ratio (0.69 ± 0.18), of the patients in the observation group were significantly higher than the (38.45 ± 5.98), (20.45 ± 6.41), (39.44 ± 3.49), and (1.31 ± 0.48), respectively, of the patients in the control group. The differences between the two groups were statistically significant (P<0.05). By contrast, the preoperative forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC), and maximum voluntary ventilation (MVV) did not significantly differ between the two groups (P>0.05). The FEV1 (1.76 ± 0.41 L), FVC (2.33 ± 0.32 L), and MVV (33.7 ± 5.2 L/min) of the observation group were significantly higher than those of the control group (P<0.05). Meanwhile, the complication rate of the observation group was 42.2%, which was significantly lower than that of the control group (75.6%) (P<0.05). -/- Conclusion: Tubular gastroesophageal anastomosis can benefit patients with esophageal cancer by significantly reducing the adverse effects on their postoperative pulmonary function and control the occurrence of postoperative complications. Abstract: Background: Coronary artery disease remains the leading cause of morbidity and mortality Worldwide. Previous reviews pointed that nursing interventions are beneficial for coronary artery patients. However, most interventions focused on education and counselling, but not consistent with the outcome set; still did not consider patient’s coronary artery disease risky characteristics. Related studies in China also difficult to find. Therefore this study was conducted to investigate kinds of nursing interventions delivered to coronary artery patients and match them with (...) patient’s risk factors of coronary artery disease. Results of this study were expected to add new knowledge that will alert nurses to consider coronary artery risk factors which in turn might enable the development of appropriate approaches to improve patient’s wellbeing hence reduce frequent coronary artery morbidity and mortality. Methods: A descriptive, cross-sectional, retrospective design using clinical case notes was employed. Study was undertaken in coronary care wards at the teaching hospital in China from November 2017 to September 2018. Structured-literature supported self-designed questionnaire was utilized for data collection. Chi square (χ2) test and multivariate logistic regression for adjusted odds ratio with 95% confidence interval were used to compare the relationship among independent (patient’s risk coronary artery disease factors) and dependent (nursing interventions) categorical variables. Ethical permission was granted accordingly. Results: A total of 300 coronary artery patients’ case notes were audited with mean age 63±11.2 years. Of these 175 (58.3%) were males. 126(42%) were smoking and 224(74.7%) were hypertensive. More evidence based nursing interventions than education and counselling were found to be delivered to these patients. “Administer coronary artery disease medication and their instructions” was mostly delivered to many patients 291(97%) while “counsel to cope with stress” was the least one 60 (20.0%). Three of eight nursing interventions delivered significantly matched with three or all of these patient’s coronary artery risk variables (age, smoking, hypertension and diabetes) (p < 0.05 and/or < 0.01) with Adjusted odds ratio (95% CI) within their significant ranges. Conclusion: This study delivers valuable insight that, nurses in the studied teaching hospital delivered beneficial evidence based nursing interventions to patients with coronary artery disease which significantly matched with their risk factors of coronary artery illness. However, care for stress was low hence needs improvement. Furthermore, research is needed to get consistency of nursing interventions with patient’s end point clinical outcomes for further appraisal of nursing efforts in caring CAD patients . (shrink)

The International Severe Acute Respiratory and Emerging Infection Consortium (ISARIC) COVID-19 dataset is one of the largest international databases of prospectively collected clinical data on people hospitalized with COVID-19. This dataset was compiled during the COVID-19 pandemic by a network of hospitals that collect data using the ISARIC-World Health Organization Clinical Characterization Protocol and data tools. The database includes data from more than 705,000 patients, collected in more than 60 countries and 1,500 centres worldwide. Patient data are available from (...) acute hospital admissions with COVID-19 and outpatient follow-ups. The data include signs and symptoms, pre-existing comorbidities, vital signs, chronic and acute treatments, complications, dates of hospitalization and discharge, mortality, viral strains, vaccination status, and other data. Deep Brain Stimulation (DBS) is a relatively new, experimental treatment for patients suffering from treatment-refractory Obsessive Compulsive Disorder (OCD). The effects of treatment are typically assessed with psychopathological scales that measure the amount of symptoms. However, clinical experience indicates that the effects of DBS are not limited to symptoms only: patients for instance report changes in perception, feeling stronger and more confident, and doing things unreflectively. Our aim is to get a better overview of the whole variety of (...) changes that OCD patients experience during DBS treatment. For that purpose we conducted in-depth, semi-structured interviews with 18 OCD patients. In this paper, we present the results from this qualitative study.We list the changes grouped in four domains: with regard to (a) person, (b) (social) world, (c)characteristics of person-world interactions, and (d) existential stance. We subsequently provide an interpretation of these results. In particular, we suggest that many of these changes can be seen as different expressions of the same process; namely that the experience of anxiety and tension gives way to an increased basic trust and increased reliance on one’s abilities. We then discuss the clinical implications of our findings, especially with regard to properly informing patients of what they can expect from treatment, the usefulness of including CBT in treatment, and the limitations of current measures of treatment success. We end by making several concrete suggestions for further research. (shrink)

There has been a significant increase in the use of radiological services in the past 30 years. There are many reasons for this, but one has received little attention: the increased role of patient autonomy in healthcare. Patients demand x rays, CT scans, MRI, and positron emission tomography scans. The key question in this article is how a moral principle, such as respect for patient autonomy, can influence the extension of radiological services. A literature review reveals how patient autonomy (...) is acknowledged in radiology, and how it is used both to explain and to justify the increase in radiological examinations. Furthermore, it also shows how the premises favouring patients’ exercise of their autonomy are not always present, which makes patient autonomy subject to adverse side effects and even abuse. Patient autonomy can be used to reduce the professionals’ responsibility for radiological examinations , to increase the popularity of the profession , to increase the income of the professionals or their institutions, and to promote professional activity. Patient autonomy intended to reduce paternalism, to legitimise otherwise morally unjustifiable actions , and to protect patients, can easily be used as a moral means for opposite ends. These adverse effects are not peculiar to radiology. However, they emerge particularly clearly in explanations and justifications of the substantial increase in radiological services, as well as in debates on overuse of radiological services. (shrink)
