Humanism in nursing: the emergence of the lightThis paper examines Western nursing practices by focusing on their spiritual aspect. The transformation of the informal and poorly trained nurse into the trained and uniform persona of the modern nurse is the subject of many nursing histories and part of nursing mythology. Using the work of Michel Foucault and Marcel Mauss, the nursing that preceded the 19th century reformers is re‐examined and continuities between current and quite ancient practices of nursing are explored. (...) The development of practices or technologies for care of the sick originated with the establishment of hospitals in the 4th century as part of pious Christian practice. Current practices of care and the discourse of holistic nursing are argued to have grown from these traditional Christian technologies of care. Humanist or holistic nursing represents the enshrinement of the Christian edios – freed of doctrine; the discourse of caring a redrafted Christian discipline of love. (shrink)

A cross‐national study of differences in the identities of nursing in England and Australia and how this has affected nurses’ capacity to respond to hospital reform This paper examines similarities and differences in the identity of nursing in England and Australia. In doing this we examine how in each country nursing has developed different ideologies and strategies. Our analysis draws on data derived from a cross‐national study of hospital staff in England and Australia. We demonstrate how differences in the occupational (...) identity of nursing in each country are registered in their values, attitudes and beliefs. We suggest that these differences reflect the interplay of factors such as the strategic stances of nursing professional and industrial organisations and how nurses in each country have positioned themselves in responding to recent changes in health policy. In the last decade, nurse scholars have focused extensively on the nature of nursing and its relationship to art and science. This emphasis has also been accompanied by an increasing literature on nursing ethics. In spite of this growing interest, the relationship of nursing art and nursing ethics has been left unclear. This paper proposes that nursing must be considered as a practical art because this conception explicates the relationship of nursing art and nursing ethics. It is based on the (...) thesis that nursing art is subordinated to nursing ethics and as such is a moral art. Different conceptions of nursing art (craft and fine art) will be explored to show their limits. This will be followed by the conception of nursing art as a practical art. (shrink)

Abstract The strength of a discipline is reflected in the development of a set of concepts relevant to its practice domain. As an evolving professional discipline, nursing requires further development in this respect. Over the past two decades in North America there have emerged three different approaches to concept analysis in nursing scholarship: Wilsonian-derived, evolutionary, and pragmatic utility. The present paper compares and contrasts these three methods of concept in terms of purpose, procedures, philosophical underpinnings, limitations, guidance for researchers, and (...) ability to contribute to nursing knowledge and disciplinary advancement. This work extends prior criticisms of concept analysis methods, especially as formulated by Morse and colleagues, by promoting further critical discussion regarding the direction and effectiveness of nursing efforts to meet the basic needs of disciplinary development. Its central thesis is that nursing concept analysis must advance beyond the Wilsonian-derived methods of Walker and Avant by devoting greater attention to understanding the domain of concepts to be analysed and deriving features from these contexts. While health promotion is widely acknowledged as a practice field where multidisciplinary teamwork is important, within nursing's discipline‐specific literature, a strong argument can be discerned regarding the profession's belief that it has a clear and unique role to play in that field. Yet rarely is this unique role, how it arises, and specifically how its effects are to be demarcated, attended to within the discipline‐specific literature. Two philosophical perspectives on science are presented and we demonstrate the extent to which these (...) two perspectives influence nursing scholarship, including nursing practice within the field of health promotion. We then go on to argue that, for nurses to sustain their claim to a unique and important contribution within health promotion, clear articulation of the philosophical premises underpinning practice methodologies is warranted. Specifically we argue the importance of such clarity within the context of an analysis of the discourse of multidisciplinary practice as a strategy for avoiding confronting the ways in which bio‐medical authority has already significantly demarcated how health promotion practice can proceed. Nicht erst seit Ruth Macklins einflussreichem Aufsatz „Dignity is a useless concept“ ist eine breite Debatte um Sinn und Unsinn des Begriffs der Menschenwürde in der Medizinethik entbrannt. In diesem Beitrag möchte ich mein Augenmerk auf die von Macklin prominent gemachte Substitutionsthese richten. In ihrer einflussreich gewordenen Fassung propagiert diese eine Ersetzung des Menschenwürdebegriffs durch denjenigen der Autonomie. Doch was ist damit genau gemeint? – Ausgehend von einer Analyse von Macklins Argumenten möchte ich zeigen, unter welchen Bedingungen die Substitutionsthese die (...) von ihren Vertretern häufig in den Vordergrund gestellten eleminativen Ergebnisse zeitigt – die Rede von einem eigenständigen begrifflichen Gehalt von „Würde“ sollte aufgegeben werden. Im Verlauf der Untersuchung werden auch die Vorzüge einer solchen Position gegenüber stärker eliminativ auftretenden Standpunkten und Positionen deutlich, die den Ausdruck als berechtigt, aber unanalysierbar betrachten. This study was designed to explore the concept of witnessed resuscitation. This was achieved through a serial approach to conceptually based research that systematically and incrementally developed understanding of the meaning of witnessed resuscitation in the context of emergency resuscitative care for adult victims of cardiorespiratory arrest. Theoretical investigation provided a strong conceptual foundation of existing knowledge and gave direction for further inquiry. Existential investigation comprised a hermeneuticphenomenological study to explore the phenomenon of lay presence during an adult cardiopulmonary resuscitation (...) attempt. Lived-experience material was subjected to thematic analysis and was revealing of five concepts that represented the essential nature of the lived experience. The concept of exposure emerged as the essence of this phenomenon. Research findings derived during theoretical and existential investigation were compared by adapting a method of template comparison. This process culminated in a synthesised conceptualisation of the meaning of witnessed resuscitation of a higher level of abstraction. Ongoing research is needed to determine whether this ‘state of the art’ conceptualisation of witnessed resuscitation holds its boundaries when applied to alternative phenomena, contexts and disciplines. Priority should be given to exploring the application of this concept in the context of patient and family-centred end-of-life care. (shrink)

The intent of this research was to explore the phenomenon of remote area nursing in relation to a theoretical framework, the SUN Conceptual Model. A Theory-Research-Theory strategy was chosen in order to modify, refine or redevelop the model. Using a descriptive, interpretive design, a sample of eight Remote Area Nurses (RANs) in Western Australia were asked to describe their experiences of remote area nursing. The data were collected by telephone interview. Themes were extrapolated and categorised according to key concepts of (...) the previously developed model. Common components of significant statements were identified in order to gain understanding of the meanings of responses. Analysis revealed that some overlap existed between the original key concepts of the model which were subsequently regrouped or made redundant. This process culminated in a revision of the original model. The revised SUN Conceptual Model was thus considered to be a useful theoretical framework for reflecting the reality of remote area nursing, as supported by the literature. Study findings point to the need for more research to further examine the model as a theoretical framework for RAN practice and to identify factors which contribute to the effectiveness of RANs. The study provides a contribution to the knowledge base of RANs while moving towards the development of a theoretical framework which may be included in education programmes and ultimately practice.
