There seem to be some misunderstandings abroad in the literature about medical epistemology and person-centered medicine concerning the nature of 'modest' or aetiological foundationalism, and some vagueness about 'emergence'. This paper urges a greater tolerance for a modest, Humean variety of foundationalism, not least because it seems to offer significant support for person-centred medicine. It also suggests a closer examination of emergence as an explanation or justification for medicine, since emergence is a complex concept that does nothing to rule out (...) foundationalism. Foundationalism has been condemned tout court without recognizing that it comes in various styles and degrees. Emergence has been used as a property of medicine to dismiss any need to seek its foundations. But emergence too comes in different forms, not all of them at odds with modest foundationalism. Modest or aetiological foundationalism is far more flexible than Cartesian foundationalism, and is compatible with modern understandings of emergence. Modest foundationalism and a particular understanding of emergence support the ontological and epistemological underpinning of person-centered medicine. (shrink)

This paper develops a phenomenological approach to the concept of pain, which highlights the main presuppositions that underlie pain research undertaken both in the natural and in the sociohistorical sciences. My argument is composed of four steps: only if pain is a stratified experience can it become a legitimate theme in both natural and sociohistorical sciences; the phenomenological method is supremely well suited to disclose the different strata of pain experience; the phenomenological account offered here identifies three fundamental levels that (...) make up the texture of pain experience: pain can be conceived as a prereflective experience, as an object of affective reflection, or as an object of cognitive reflection; and such a stratified account clarifies how pain can be a subject matter in the natural and sociohistorical sciences. Arguably, the natural and sociohistorical sciences address pain at different levels of its manifestation. While the natural sciences address pain as an object of cognitive reflection, sociohistorical sciences first and foremost deal with pain as a prereflective experience and as an object of affective reflection. As a singular witness and actor of the tumultuous 20th century, Ernst Jünger remains a controversial and enigmatic figure known above all for his vivid autobiographical accounts of experience in the trenches of the First World War. This article will argue that throughout his entire oeuvre, from personal diaries to novels and essays, he never ceased to grapple with what he viewed as the central question of the age, namely that of the problem of nihilism and the means to overcome (...) it. Inherited from Nietzsche’s diagnosis of Western civilization in the late 19th century, to which he added an acute observation of the particular role of technology within it, Jünger would employ this lens to make sense of the seemingly absurd industrial slaughter of modern war and herald the advent of a new voluntarist and bellicist order that was to imminently sweep away timorous and decadent bourgeois societies obsessed with security and self-preservation. Jünger would ultimately see his expectations dashed, including by the forms of rule that National Socialism would take, and eventually retreated into a reclusive quietism. Yet he never abandoned his central problematique of nihilism, developing it further in exchanges with Martin Heidegger after the Second World War. And for all the ways in which he may have erred, his life-long struggle with meaning in the age of technique and its implications for war and security continues to make Jünger a valuable interlocutor of the present.
