This article presents an ethnographical and historical analysis of the mode of being that is constituted when people use neurofeedback for self-improvement. I analyse how human brainwaves have been associated with the psyche since their first demonstration by the psychiatrist Hans Berger, how they were connected to personality types by the cybernetician Grey Walter, and made trainable by the psychologists Joe Kamiya and Barry Sterman. I compare these cases with the reports of contemporary neurofeedback practitioners and users, and demonstrate that (...) working on the self by working on the brain constitutes a complicated relationship between the brain and the self. Moreover, I demonstrate that combinations of brains and selves, material and spiritual ideas, and biological and social explanations are not confusions due to the ignorance of neurofeedback users, but amalgamations that emerged in the work and ideas of early scientists. Electroencephalography (EEG) neurofeedback is a type of biofeedback that purportedly teaches users how to control their brainwaves. Although neurofeedback is currently offered by thousands of providers worldwide, its provision is contested, as its effectiveness beyond a placebo effect is unproven. While scholars have voiced numerous ethical concerns about neurofeedback—regarding opportunity cost, physical and psychological harms, financial cost, and informed consent—to date these concerns have remained theoretical. This pilot study aimed to provide insights on whether these issues were supported by empirical (...) data from the experiences of neurofeedback users. Semi-structured telephone interviews were conducted with individuals who had used EEG neurofeedback for themselves and/or for a child. The majority of respondents (N = 36) were female (75%), white (92%), and of higher socioeconomic status relative to the U.S. population. Among adult users (n = 33), most (78.8%) resorted to neurofeedback after having tried other therapies, and were satisfied with the treatment (81.8%). The majority paid for neurofeedback out-of-pocket (72.7%) and considered it to be good value for money (84.8%). More than half (57.6%) considered neurofeedback to be a scientifically well-established therapy. However, of those, 78.9% were using neurofeedback for indications not adequately supported by scientific evidence. Concerns regarding opportunity cost, physical and psychological harms, and financial cost are not substantiated by our findings. Our results partially support concerns regarding insufficient understanding of limitations. This study underlines the disconnect between some of the theoretical concerns raised by scholars regarding the use of non-validated therapies and the lived experiences of users. (shrink)

Nearly all research in neurofeedback since the 1960s has focused on training voluntary control over EEG constructs. By contrast, EEG state discrimination training focuses on awareness of subjective correlates of EEG states. This study presents the first successful replication of EEG alpha state discrimination first reported by Kamiya . A 150-s baseline was recorded in 106 participants. During the task, low triggered a prompt. Participants indicated “high” or “low” with a keypress response and received immediate feedback. Seventy-five percent of participants (...) achieved significant discrimination within nine sessions, with a significant learning curve effect. Performance was significantly related to physical properties of the EEG signal, including magnitude, duration, and absolute vs. relative amplitude. These results are consistent with a conceptualization of EEG state discrimination as a sensory modality, although it is also intricately related to voluntary control of these states. (shrink)

Currently there are several psychotherapy modalities utilising theory and research from neuroscience in treatment frameworks for mental health and recovery from trauma. In Australia this includes: the Conversational Model of Psychodynamic Psychotherapy, a contemporary psychodynamic approach used for treating Borderline Personality Disorder and other trauma-related disorders; Electroencephalogram Neurofeedback, a brain training therapy which has been used as an adjunct to counselling/psychotherapy in traumatic stress and developmental trauma; and Somatic Experiencing, an integrative mind-body approach based on body responses to threat and (...) fear, especially thwarted attempts to enact fight or flight in the face of threat. These modalities have a promising but still-limited evidence base. This paper explores the role of different types of evidence to frame a descriptive review of the current evidence base available for clinicians practicing in these three modalities. While large clinical trials can provide evidence of comparative effectiveness, case studies, observational, qualitative and practice-based enquiry can assist in discovering other aspects of therapy important for individual clients and practitioners. Existing frameworks of evidence- and ethics-informed health and clinical decision-making suggest an ethical approach can incorporate new, developing and evidence-building therapies. Recommendations include careful consideration of informed consent for therapy including disclosure of efficacy and safety, and specific to psychotherapies incorporating neuroscience, a need for practitioners to reflectively assess their own knowledge, competence, heuristic approaches and biases.
