Many blame politicians, governments, and markets for the technically-driven problems the world faces. But why is it that there are almost always engineers and corporations willing to design and build the technologies that cause those problems, many times in spite of knowing about the negative consequences of those technologies? I offer in this paper practical guidance on how to engage in activist engineering, the goal of which is to get engineers to step back from their work and be able to (...) ask and have a conversation about the question, “What is the real problem, and does this problem ‘require’ an engineering solution?” Building on research in the history and philosophy of engineering, and engineering ethics and education, as well as current events—all of which highlight important issues of debate within engineering practice—I provide a list of questions that engineers can start with for self-reflection to better understand their motivations for doing engineering work, and to better understand the implications of their work. The questions relate to considerations engineers must make regarding the social, environmental, economic, and peace implications of their work, and relate to alternative and non-technical interventions to the problem at hand. I believe that each engineer should, in the end, be able to answer the questions: Why am I an engineer? For whose benefit do I work? What is the full measure of my moral and social responsibility? This comprehensive review explores the evolving epistemological foundations of Human Reliability Analysis (HRA), tracing its transition from a practical, applied field to a scientific discipline. The paper critically examines the philosophical assumptions that underlie prominent HRA models, emphasizing their impact on both scientific validity and practical application. Organized into five sections, the paper begins by articulating its primary aim in Section 1: to uncover and scrutinize the philosophical assumptions within HRA models. Sections 2.2 and 2.3 delve into reductionist and holistic (...) paradigms. Section 2.4 focuses on the shift toward holistic theories, emphasizing the integration of human, technical, and organizational elements in modern socio-technical systems. Section 3 concludes by examining systemic approaches, capturing the paradigm shift from reductionism to a broader, interdependent view of accidents and reliability. Section 4 sheds light on the contemporary HRA models, emphasizing the prevalence of systemism in recent research. Notable models, such as SoTeRiA and FRAM, undergo scrutiny for their systemic approach, revealing interrelationships crucial for understanding accidents in complex operations. In Section 5, the paper explores the practical implications of the epistemological insights gained, emphasizing their potential impact on the development and enhancement of HRA models. The conclusion advocates for a paradigm shift in HRA that involves rethinking the epistemological bases of models. It underscores the need for ongoing philosophical scrutiny to drive the discipline's progress, ultimately enhancing our understanding, prediction, and improvement of human performance and system safety. Undoubtedly, technology continues to permeate the world at an unprecedented pace. The discipline of nursing is not alien to this phenomenon as nurses continue to employ various technological objects and applications in clinical practice, education, administration and research. Despite the centrality of technology in nursing, it has not been recognised as a metaparadigm domain of interest in the discipline of nursing. Thus, this paper sought to examine if technology truly reflected a metaparadigm domain using the four requirements posited by Fawcett. (...) Using these requirements, we examined the onto‐epistemology of technology in relation to nursing and conclude that technology potentially represents a distinct domain that intersects with nursing (particularly, from the humanities perspective). Also, technology encompasses some phenomena of interest to the discipline of nursing, demonstrates perspective‐neutrality, and is international in scope and substance albeit with some nuances which do not fit well with nursing onto‐epistemology. Put together, it is highlighted that technology intersects with the existing metaparadigm domains (person, health, environment and nursing) which positions it as a potential phenomenon of interest to the discipline of nursing requiring further work to articulate its position and role.
