It is difficult to understand the meaning of ‘dignity’ in human rights, bioethics and nursing literature because the word is used so vaguely. Unless dignity’s meaning is spelt out it can disappear beneath more tangible priorities. In this article we define dignity and show how this can help health workers to maintain the dignity of people in their care.

Declarations of the importance of dignity in health care are commonplace in codes of practice and other mission statements, yet these documents never clarify dignity’s meaning. Their vague aspirations are compared to comments from staff and patients about opportunities for and barriers against the promotion of dignity in elderly care institutions. These suggest that while nurses and health care assistants have an intuitive understanding of dignity, they either do not or cannot always bring it about in practice. Thus, despite stated (...) intentions to promote dignity, it appears that the circumstances of at least some elderly care institutions cause patients to experience avoidable indignities. Such institutions are “undignifying institutions” because they fail to acknowledge dignity’s basic components, focus excessively on quantifiable priorities, and have insufficient resources available to assure consistently dignifying care. As a partial solution, we argue that health workers should be taught to understand and specify the components of dignity, which will better prepare them to challenge undignifying practices and to recognise opportunities for dignity promotion. On 10 December 1948, the General Assembly ofthe United Nations adopted and proclaimed the Universal Declaration of Human Rights, a truly historic document, the full text of which is reproduced here. Following this historic act, the Assembly called upon all Member countries to publicize the text of the Declaration and "to cause it to be disseminated, displayed, read and expounded principally in schools and other educational institutions, without distinction based on the political status of countries or territories." This article attempts to elucidate the ethical meaning behind the words ‘the ethics of self’ and ‘an ethical self’, particularly in the light of Noddings’ ‘ethical self’, in order to show the relevance of these terms to the practice of ethical caring. It examines the relationship that Noddings believes exists between one’s actual self and the vision one has of one’s ideal self. I attempt to draw out a meaning derived from the texts in which this concept has been captured, (...) while at the same time juxtaposing the concept ‘ethic of care’ from my own research, together with the meaning given by philosophers Peta Bowden, Iris Murdoch and Simone Weil, who have written on the subject, albeit indirectly in some instances. A participant in a colleague’s research on care and patient satisfaction used similar expressions, and gave the impetus for this article. (shrink)
