These beliefs form the foundation of the "educational initiation that prepares and licenses the student for professional practice". The nature of the "rigorous and rigid" preparation helps ensure that the received beliefs are firmly fixed in the student's mind. Scientists take great pains to defend the assumption that scientists know what the world is like...To this end, "normal science" will often suppress novelties which undermine its foundations. A pluralistic view of psychiatric classification is defended, according to which psychiatric categories take a variety of structural forms. It is argued that psychiatric categories cannot all be understood as pragmatically grounded, and at least some reflect naturally occurring discontinuities without thereby representing natural kinds. We discuss Russell's 1913 essay arguing for the irrelevance of the idea of causation to science and its elimination from metaphysics as a precursor to contemporary philosophical naturalism. We show how Russell's application raises issues now receiving much attention in debates about the adequacy of such naturalism, in particular, problems related to the relationship between folk and scientific conceptual influences on metaphysics, and to the unification of a scientifically inspired worldview. In showing how to recover an approximation to Russell's conclusion (...) while explaining scientists' continuing appeal to causal ideas (without violating naturalism by philosophically correcting scientists) we illustrate a general naturalist strategy for handling problems around the unification of sciences that assume different levels of naïveté with respect to folk conceptual frameworks. We do this despite rejecting one of the premises of Russell's argument, a version of reductionism that was scientifically plausible in 1913 but is not so now. (shrink)

How ironic that my application of pragmatism to psychopathology has been charged (...) with not being sufficiently pluralistic because it tries to fit all psychiatric disorders into the Procrustean bed of practical kinds. The Practical Kinds Model in General As a point of clarification, the practical kinds model is not a description of a type of category combined with an argument that all psychiatric disorders should be seen as belonging to that type. If it were that kind of model, I would fully agree with Haslam that practical kinds, fuzzy kinds, and discrete kinds are all potential types of psychiatric disorders. Like Nick Haslam, I believe that there are many different things in the DSM-IV and the ICD-10, and they cannot neatly be classified as the same type of thing.Rather than identifying a type of category, the practical kinds model is a general theory of scientific classification. It is a nonessentialistic theory. Essentialism is the view that things have real essences. In the Aristotelian version of essentialism, an essence is an internal structure, and it is the internal structure that makes anything be the kind of thing it really is. In this classical category model, category members either have the essence or they do not, and everything that has the appropriate essence is a member of that category. The world is supposed to therefore have a well-defined structure, and the job of the scientist is to discover that structure. Ian Hacking (1999) has termed this view of categorization inherent structurism.The practical kinds model does not deny that things have internal structures; it only denies that internal structure by itself determines category membership. Internal structure is an important consideration in identifying categories, but external criteria can always play a role. External or relational criteria play more important roles in some cases than they do in others, but they cannot be eliminated once and for all. Essentialistic theories, which favor an almost exclusive focus on inherent structures, have the advantage of being more parsimonious, but they do so by ignoring real complexity.The best way to illustrate this point is to examine a category whose identification is heavily weighted in terms of internal criteria, or an inherent structure, such as a chemical element. [End Page 219] For example, hydrogen is defined as the element that has an atomic number of 1. This means that it has one proton in its nucleus and normally one electron orbiting its nucleus. The atomic number reflects an internal property, and anything that has an atomic number of 1 is hydrogen. The same is true for the other elements. Anything that has an atomic number of 79 is gold; anything that has an atomic number of 19 is potassium, and so on. Also part of the essentialistic view is the notion that all the important properties of an element are systematically (or lawfully) related to its internal structure.It would be foolish to deny the practical utility of the atomic theory of chemistry. But even in this strongest of cases, external criteria can potentially play a role in how we decide to carve up the world and what we identify as the relevant categories. The point can be clearly illustrated with a Gedanken experiment based on the ideas of Donnellan (1983). I am grateful to the two commentators for giving my paper their serious attention, and for writing such stimulating, clarifying, and challenging responses. In a brief response I can only begin to discuss a select few issues, although both commentaries could generate a great (...) deal of profitable exchange.Zachar's commentary elaborates his practical kinds model, discusses several of the graded series of kinds proposed in my taxonomy of psychiatric categories, and disputes some aspects of my treatment of natural kinds. Zachar and I have no quarrel on his first point. I recognize his practical kinds model as a general theory of psychiatric classification rather than simply as a description of one type of category, the "practical kind." It was perhaps bad form to pilfer a term from Zachar's model and give it a more circumscribed role in my own, although by doing so I intended both to acknowledge the importance of the concept and to situate it among some of its alternatives.All the same, I stand by the account of practical kinds developed in my paper, even if it reflects a narrower sense than Zachar's. In my sense, practical kinds are pragmatically drawn distinctions imposed on continuous dimensions, whereas to Zachar all psychiatric categories are at least to some degree practical. He therefore proposes a broad "continuum" of practical kinds. The difficulty that I have with this proposal is that such a continuum implies a fundamental unity among psychiatric categories: categories are all to be understood first and foremost in pragmatic terms and differences in kind between them in internal structure are to be neglected. In my view, although the pragmatic bases of psychiatric classification are vitally important, and perhaps inadequately formulated in my paper, the neglect of internal structure and of possible breaks in the continuum of kinds are real limitations of the practical kinds model. Although Zachar is partially justified in claiming that his view is pluralistic, allowing as it does for variability in degrees of practicality, by failing to acknowledge distinctly different kinds of categories in the psychiatric domain, his view is less pluralistic than I believe it should be.One example of what I take to be this neglect of structural differences is a conflation of two sorts of practicality in Zachar's work. On the one hand, certain categories such as hypertension or obesity are defined on pragmatic grounds, a binary distinction being drawn in the absence of an internal discontinuity, because individuals identified as category members are adjudged to [End Page 237] be at significantly elevated risk. On the other hand, some categories just happen to have high practical utility. Zachar argues that schizotaxia, for example, is practical because it affords a variety of scientific and clinical inferences.It seems to me that these sorts of practicality—or perhaps these sorts of relations to practicality—are importantly different. Hypertension is defined as a category because of its practical utility—because a certain blood pressure threshold marks an inflection in actuarial risk of other illnesses—and might therefore be said to have "front-end" or constitutive practicality. Schizotaxia, which taxometric research shows to be a discrete kind (Haslam, 2003), is not defined and identified primarily on the basis of its relation to an external criterion, but on the basis of its internal structure. Its features cohere in a way that indicates a category with a determinate prevalence. It has "back-end" or consequential practicality to the degree that this category enables useful predictions. These two proposed forms of practicality are equally important: both mean that categories will be pragmatically valuable. However, they are also different, being characteristic of different kinds of categories and perhaps even negatively related. This work is unique in bringing concepts and methods from cognitive anthropology to bear on psychiatric classification. At first blush, it might seem questionable to treat clinicians’ beliefs about psychiatric disorders as folk taxonomies, no different in kind from classifications of birds produced by (...) members of a forest-dwelling tribe. Unlike birds and other natural objects of folk taxonomies, mental disorders are not concrete entities and they cannot be distinguished on perceptual grounds. Unlike tribespeople, clinicians are not simply “folk” whose naïve classifications can be compared with those of scientific authorities. They are, in fact, heavily exposed to the reigning official taxonomy during their professional training, and their thinking can hardly remain unaffected by it. It is a testament to Flanagan and Blashfield’s work that despite these apparent disanalogies, their folk-taxonomic approach manages to illuminate how clinicians carve up the psychiatric domain.This paper reports some careful, creative, and labor-intensive studies to make a few fundamental claims about how clinicians classify. First, the authors argue that clinicians hold simplified taxonomies compared with the official DSM-IV taxonomy: They encompass fewer disorders and classify them in a shallower hierarchy. Second, they claim that clinicians substantially agree in how they classify disorders, and that a consensus classification can therefore be extracted from their individual taxonomies. Third, they argue that clinicians’ taxonomies show only a modest consistency with DSM-IV, and that the discrepancies may be systematic and important. Notably, for example, clinicians did not preserve the fundamental divide between the DSM’s Axes I and II. My comments address the second and third claims.Is There a “Consensus” among the Clinicians?One of the main achievements of Flanagan and Blashfield’s paper is to develop a “consensual taxonomy,” which is laid out in their Table 2a. This taxonomy clearly distils how their clinicians tended to judge the relations among diagnoses. However, it is an exaggeration to present it as a consensus. If, by Romney, Weller, and Batchelder’s (1986) third criterion, “cultural consensus” requires that the primary factor in a principal components analysis accounts for “most” of the variance in clinicians’ judgments, then Flanagan and Blashfield’s analysis [End Page 281] fails to show consensus. Their first factor accounts for only twenty-five percent of the variance, well short of the required majority. This finding implies that there is three times as much inconsistency or variability among clinicians’ taxonomic judgments than there is consistency and commonality. The observed factor-analytic findings are compatible with the average correlation between pairs of clinicians being only about 0.2, a very weak level of agreement.This is not to say that there is no consistency among the clinicians, or that there is no value in summarizing that consistency, but it does indicate that there is a great deal of diversity as well. Clinicians’ folk taxonomies seem to be highly idiosyncratic, more a Babel of distinctive classifications than a chorus of agreement. It is therefore risky to draw strong generalizations about how clinicians classify, and to imply that there is a single consensual taxonomy.Several things are very interesting about this diversity of classifications. First, Flanagan and Blashfield show that it cannot be explained by the clinicians’ differing levels of experience or by the different methodologies used to elicit their taxonomies. The variability among clinicians must therefore have some other basis, which awaits further investigation. Second, the level of agreement (“consensus”) obtained here is significantly lower than what has been found in more orthodox studies of folk taxonomy. By implication, the classification of psychiatric disorders is a slipperier enterprise than the classification of animals and plants. People who have extensive professional experience and training in the field of psychopathology—vocational training that includes the standardizing forces of accrediting bodies and a dominant psychiatric nosology—nevertheless tend not to agree substantially about how to map that territory.
