BackgroundClinical ethics consultations can be complex interventions, involving multiple methods, stakeholders, and competing ethical values. Despite longstanding calls for rigorous evaluation in the field, progress has been limited. The Medical Research Council proposed guidelines for evaluating the effectiveness of complex interventions. The evaluation of CEC may benefit from application of the MRC framework to advance the transparency and methodological rigor of this field. A first step is to understand the outcomes measured in evaluations of CEC in healthcare settings. ObjectiveThe primary (...) objective of this review was to identify and map the outcomes reported in primary studies of CEC. The secondary objective was to provide a comprehensive overview of CEC structures, processes, and roles to enhance understanding and to inform standardization.MethodsWe searched electronic databases to identify primary studies of CEC involving patients, substitute decision-makers and/or family members, clinicians, healthcare staff and leaders. Outcomes were mapped across five conceptual domains as identified a priori based on our clinical ethics experience and preliminary literature searches and revised based on our emerging interpretation of the data. These domains included personal factors, process factors, clinical factors, quality, and resource factors. ResultsForty-eight studies were included in the review. Studies were highly heterogeneous and varied considerably regarding format and process of ethical intervention, credentials of interventionist, population of study, outcomes reported, and measures employed. In addition, few studies used validated measurement tools. The top three outcome domains that studies reported on were quality, process factors, and clinical factors. The majority of studies examined multiple outcome domains. All five outcome domains were multidimensional and included a variety of subthemes.ConclusionsThis scoping review represents the initial phase of mapping the outcomes reported in primary studies of CEC and identifying gaps in the evidence. The confirmed lack of standardization represents a hindrance to the provision of high quality intervention and CEC scientific progress. Insights gained can inform the development of a core outcome set to standardize outcome measures in CEC evaluation research and enable scientifically rigorous efficacy trials of CEC. (shrink)

ZusammenfassungWährend die Implementierung und Nutzung von Ethikberatung in deutschen Krankenhäusern in den letzten 20 Jahren vorangeschritten ist, entstehen erst in den letzten Jahren zunehmend Ethikberatungsangebote im außerklinischen Bereich. In den Landkreisen Traunstein und Berchtesgadener Land in Südost-Bayern wurde 2012 eine außerklinische Ethikberatung etabliert. Um im Rahmen einer ersten Evaluation wissenschaftliche Erkenntnisse über den Bedarf, die Inanspruchnahme und die Zufriedenheit mit diesem Beratungsangebot zu gewinnen, wurde eine Dokumentenanalyse und eine Fragebogenerhebung unter den Hausärzten der beiden Landkreise durchgeführt. Insgesamt nahmen 58 der (...) 195 angeschriebenen Hausärzte an der Umfrage teil.Die Auswertung der Dokumentation der außerklinischen Ethikberatung zeigte, dass die meisten Anfragen von betreuenden Angehörigen und aus Pflegeheimen kamen. Den Ergebnissen der Fragebogenerhebung zufolge sahen sich die Hausärzte mit vielfältigen ethischen Fragestellungen konfrontiert. Das Angebot einer ethischen Fallbesprechung zur Lösung dieser Konflikte wurde bislang jedoch nur von 17 Hausärzten in Anspruch genommen. Als Hauptgründe für die fehlende Nutzung des Angebots wurde der mangelnde Bekanntheitsgrad, der fehlende Bedarf und der organisatorische bzw. zeitliche Aufwand angeführt. Gleichzeitig äußerten Hausärzte, die eine ethische Fallbesprechung in Anspruch genommen haben, zu einem Großteil Interesse an einer erneuten Beratung und empfanden das Ergebnis der Ethikberatung als hilfreich. Damit verdeutlichen die Ergebnisse der Studie, dass Hausärzte zwar vielfältige ethische Fragestellungen in ihrem Arbeitsalltag wahrnehmen, aber bislang nur vergleichsweise selten ein externes Beratungsangebot zur Entscheidungsunterstützung in Betracht ziehen. BackgroundAmidst expanding roles in education and policy making, questions have been raised about the ability of Clinical Ethics Committees (CEC) s to carry out effective ethics consultations (CECons). However recent reviews of CECs suggest that there is no uniformity to CECons and no effective means of assessing the quality of CECons. To address this gap a systematic scoping review of prevailing tools used to assess CECons was performed to foreground and guide the design of a tool to evaluate the quality (...) of CECons.MethodsGuided by Levac et al’s (2010) methodological framework for conducting scoping reviews, the research team performed independent literature reviews of accounts of assessments of CECons published in six databases. The included articles were independently analyzed using content and thematic analysis to enhance the validity of the findings.ResultsNine thousand sixty-six abstracts were identified, 617 full-text articles were reviewed, 104 articles were analyzed and four themes were identified – the purpose of the CECons evaluation, the various domains assessed, the methods of assessment used and the long-term impact of these evaluations.ConclusionThis review found prevailing assessments of CECons to be piecemeal due to variable goals, contextual factors and practical limitations. The diversity in domains assessed and tools used foregrounds the lack of minimum standards upheld to ensure baseline efficacy.To advance a contextually appropriate, culturally sensitive, program specific assessment tool to assess CECons, clear structural and competency guidelines must be established in the curation of CECons programs, to evaluate their true efficacy and maintain clinical, legal and ethical standards. ZusammenfassungEthik-Cafés in der geriatrischen Langzeitpflege dienen auch der ethischen Weiterbildung und sollen den Beteiligten in ungezwungener Atmosphäre einen offenen Diskurs über moralische Fragen mit Bezug zum Arbeitsalltag ermöglichen. In der Literatur werden ihnen diverse Eigenschaften wie Verbesserung der Analysefähigkeit und der Kommunikation, Erhöhung der ethischen Sensibilität und der Entscheidungskompetenz und weitere zugeschrieben. Diese Eigenschaften resultieren vorwiegend aus theoriegeleiteten Modellen. Gegenstand dieser Studie ist die empirische Untersuchung der Wirksamkeit von Ethik-Cafés innerhalb der Pflegezentren Mattenhof-Irchelpark, Zürich. Des Weiteren geht die Studie der (...) Frage nach, ob sich die subjektiven Einschätzungen der Eigenschaften von Pflegenden und anderen Berufsgruppen unterscheiden. Zur Erfassung der Eigenschaften wurde ein Fragebogen konstruiert, in dem die Teilnehmenden während der Erhebungszeit 16 Aussagen zu bewerten hatten. Die Bewertung konnte mittels einer 5-stufigen Likert-Skala angegeben werden. In einer Untergruppe wurde zwischen „Pflege“ und „Nicht-Pflege“ unterschieden. Zusammenfassend konnte festgestellt werden, dass die postulierten Eigenschaften mit einer sehr hohen Zustimmung bestätigt werden. Die Ethik-Cafés leisten nach Meinung der Befragten einen wichtigen Beitrag zur Befähigung des Personals, schwierige Situationen klarer zu analysieren, ethisch sensibler zu entscheiden und die getroffene Entscheidung zu begründen. Die Interdisziplinarität der Ethik-Cafés fördert das Verständnis für die Sichtweisen und Argumentationen der verschiedenen Bereiche und Hierarchien. Die Unterschiede der Berufsgruppen sind teilweise hoch-signifikant. Es kann festgehalten werden, dass die im Fragebogen attribuierten Eigenschaften von der Pflege generell stärker und bedeutsamer wahrgenommen werden. (shrink)

The interdisciplinary Hannover Qualification Programme on ethics consultation has trained hospital staff to operate clinical ethics services in their respective hospitals since 2003. To evaluate Hannover Qualification Programme, all former participants were contacted using an online questionnaire including four domains: status quo before attending Hannover Qualification Programme, present status, impact of Hannover Qualification Programme, future challenges. Research objectives were the long-term satisfaction with Hannover Qualification Programme and its impact on clinical ethics services. The response rate was 45%. Hannover Qualification Programme (...) was evaluated as helpful and the responders were capable of applying their acquired skills. Most participants could contribute to the implementation of clinical ethics services. They were satisfied with Hannover Qualification Programme and with the degree of changes in their hospitals. In conclusion, clinical ethics education had long-term effects on trainees and on their respective hospitals. Problems were mentioned rather in the field of utilisation than with implementation or quality of clinical ethics services. (shrink)

The growing prevalence of health care ethics consultation (HCEC) services in the U.S. has been accompanied by an increase in calls for accountability and quality assurance, and for the debates surrounding why and how HCEC is evaluated. The objective of this study was to evaluate the effectiveness of HCEC as indicated by several novel outcome measurements in East Asian medical encounters.

As euthanasia has become a widely debated issue in many Western countries, hospitals and nursing homes especially are increasingly being confronted with this ethically sensitive societal issue. The focus of this paper is how healthcare institutions can deal with euthanasia requests on an organizational level by means of a written institutional ethics policy. The general aim is to make a critical analysis whether these policies can be considered as organizational-ethical instruments that support healthcare institutions to take their institutional responsibility for (...) dealing with euthanasia requests. By means of an interpretative analysis, we conducted a process of reinterpretation of results of former Belgian empirical studies on written institutional ethics policies on euthanasia in dialogue with the existing international literature. The study findings revealed that legal regulations, ethical and care-oriented aspects strongly affected the development, the content, and the impact of written institutional ethics policies on euthanasia. Hence, these three cornerstones—law, care and ethics—constituted the basis for the empirical-based organizational-ethical framework for written institutional ethics policies on euthanasia that is presented in this paper. However, having a euthanasia policy does not automatically lead to more legal transparency, or to a more professional and ethical care practice. The study findings suggest that the development and implementation of an ethics policy on euthanasia as an organizational-ethical instrument should be considered as a dynamic process. Administrators and ethics committees must take responsibility to actively create an ethical climate supporting care providers who have to deal with ethical dilemmas in their practice. (shrink)
