Gender, ‘race’, poverty, health and discourses of health reform in the context of globalization: a postcolonial feminist perspective in policy researchIn this paper, I draw on extant literature and my empirical work to discuss the impact of globalization and healthcare reform on the lives of women — those from countries of the South as well as of the North. First, I review briefly the economic hardships identified in different sectors of the population that have been attributed to how globalization is (...) now working. Second, I examine what these global processes mean for health, with particular focus on poverty, gender, racialization and health. Third, I reflect on how nurse scientists might develop research agendas in the 21st century that would foster social transformation and social justice for all people. The position taken here is not an indictment of globalization. Rather, I argue that globalization is a fact in all of our lives. There are positive aspects of globalization. There are also negative aspects which we must collectively address, given that the issues identified can have deleterious consequences for the world’s poor, women in particular. I suggest that, to construct knowledge for practice and praxis, research agendas of the future should be inclusive of subaltern voices. I argue that drawing on a postcolonial feminist epistemology might help us to define such agendas, and express the multilayered sociopolitical contexts of health and illness in advocacy with policy‐makers. Beginning with women's experience, the author examines the field's actual practices of reasoning and conceptualization. She argues that standard sociological methods of inquiry make use of ideological practices, transforming the actualities of people's lives into a formalized picture lacking subjects and subjectivity. The method of Smith recommends anchors a Marxist materialism, based in people's activities, to a woman's stand-point based in experience. She uses this method in a radically original way to explore ideology and objectified knowledge as the conceptual practices (...) of ruling. Smith is equally concerned with the application of sociological ideology to the human service bureacracy and the way institutions of mental health reconstruct women's lives. She provides meticulous accounts of the ways in which police reports, government statistics, hospital records, and pschiatric files are ideologically interpreted, transforming a person's life history in the process. In a revelatory chapter on the biographer Quentin Bell's account of Virginia Woolf's suicide, the author demonstrates how the text implicates the reader in the objectification of Woolf's "psychiatric problems." Highly critical of current sociological practices, The Conceptual Practices of Power both recommends and exemplifies the alternative approach that Smith presented in her earlier work, That Everyday World as Problematic, also published by Northeastern University Press. Nursing as normative praxisThe purpose of this paper is twofold. First, it introduces a variety of concepts of ‘praxis’, and argues in support of those which reflect the normative dimension of the critical social perspective. This begins with the Aristotelian concept, and moves through a variety of sources, including Hannah Arendt and Paulo Freire, but focuses primarily, and uniquely in the nursing literature, upon the work of the Yugoslavian ‘praxis Marxists’. Second, specific ways of conceiving nursing as praxis are outlined, (...) including political, aesthetic and ethical forms which have immediate import for nurses. The paper concludes with brief suggestions as to how these ways of conceiving praxis may be used by nurses to develop a more intimate and productive understanding of their own practices. (shrink)

Nursing as a profession seems to avoid considering the problem of racism. There is, however, a need to address this topic and to evaluate its implications for nursing practice. This article attempts to establish a rationale for nursing to address racism and introduce it into academic discourse. The results of a small-scale study by the author are analysed and the implications for ethical-moral reasoning in nursing practice are discussed in relation to professional codes of conduct developed by nurses’ professional organizations (...) in the UK and elsewhere. Poststructuralism and nursing: uncomfortable bedfellows?The benefits and limitations of the application of poststructuralist in nursing research are discussed. The debate concerning the use of poststructuralist theory in feminist research is drawn on to argue a divergence between a deconstructionist poststructuralism and nursing aims. It is argued that there are strong parallels between nursing and social movements such as feminism. The reasons why many feminist and nursing researchers have been attracted to poststructuralist theory are explored, as are the criticisms of poststructuralism (...) as an inadequate, even reactionary theory. It is argued that poststructuralism is useful for examining or deconstructing existing discourse and practice, but incompatible and inadequate for research which aims to develop or change such discourses or practices.
