Deep Brain Stimulation (DBS) is a relatively new, experimental treatment for patients suffering from treatment-refractory Obsessive Compulsive Disorder (OCD). The effects of treatment are typically assessed with psychopathological scales that measure the amount of symptoms. However, clinical experience indicates that the effects of DBS are not limited to symptoms only: patients for instance report changes in perception, feeling stronger and more confident, and doing things unreflectively. Our aim is to get a better overview of the whole variety of changes that (...) OCD patients experience during DBS treatment. For that purpose we conducted in-depth, semi-structured interviews with 18 OCD patients. In this paper, we present the results from this qualitative study.We list the changes grouped in four domains: with regard to (a) person, (b) (social) world, (c)characteristics of person-world interactions, and (d) existential stance. We subsequently provide an interpretation of these results. In particular, we suggest that many of these changes can be seen as different expressions of the same process; namely that the experience of anxiety and tension gives way to an increased basic trust and increased reliance on one’s abilities. We then discuss the clinical implications of our findings, especially with regard to properly informing patients of what they can expect from treatment, the usefulness of including CBT in treatment, and the limitations of current measures of treatment success. I offer a new metacognitive account of the feeling of agency over bodily actions. On this model the feeling of agency is the metacognitive monitoring of two cues: i) smoothness of action: done via monitoring the output of the comparison between actual and predicted sensory consequences of action and ii) action outcome: done via monitoring the outcome of action and its success relative to a prior intention. Previous research has shown that the comparator model offers a powerful explanation of the (...) feeling of agency. However, within the literature there is a growing sense of unease with the model; a consensus seems to be building that the model is not up to the task of explaining all of the new discoveries made regarding the feeling of agency since its inception. Most problematically there are paradigms which deeply challenge the comparator model by suggesting that a weak feeling of agency can be elicited even when no motor prediction is formed. The new account offered here inherits the explanatory power of the comparator model whilst avoiding this problem. (shrink)

Sense of agency is a compelling but fragile experience that is augmented or attenuated by internal signals and by external cues. A disruption in SoA may characterise individual symptoms of mental illness such as delusions of control. Indeed, it has been argued that generic SoA disturbances may lie at the heart of delusions and hallucinations that characterise schizophrenia. A clearer understanding of how sensorimotor, perceptual and environmental cues complement, or compete with, each other in engendering SoA may prove valuable in (...) deepening our understanding the agency disruptions that characterise certain focal neurological disorders and mental illnesses. Here we examine the integration of SoA cues in health and illness, describing a simple framework of this integration based on Bayesian principles. We are perceivers, we are thinkers, and we are also agents, bringing about physical events, such as bodily movements and their consequences. What we do tells us, and others, a lot about who we are. On the one hand, who we are determines what we do. On the other hand, acting is also a process of self-discovery and self-shaping. There is an increasing amount of empirical work investigating the sense of agency, i.e. the registration that we are the initiators of our own actions. Many studies try to relate the sense of agency to an internal feed-forward mechanism, called the ‘‘comparator model’’. In this paper, we draw a sharp distinction between a non-conceptual level of feeling of agency and a conceptual level of judgement of agency. By analyzing recent empirical studies, we show that the comparator model is not able (...) to explain either. Rather, we argue for a two-step account: a multifactorial weighting process of different agency indicators accounts for the feeling of agency, which is, in a second step, further processed by conceptual modules to form an attribution judgement. This new framework is then applied to disruptions of agency in schizophrenia, for which the comparator model also fails. Two further extensions are discussed: We show that the comparator model can neither be extended to account for the sense of ownership (which also has to be differentiated into a feeling and a judgement of ownership) nor for the sense of agency for thoughts. Our framework, however, is able to provide a unified account for the sense of agency for both actions and thoughts. (shrink)

This chapter argues that the experience of authorship strongly depends on the capacity to form predictions about the outcome of an action. It reviews cognitive psychology and neuroscience research indicating that subjective agency experience depends on the integrity of outcome predictions generated by internal forward models in the brain. Outcome predictions can be used for filtering sensory input through a continuous comparison with the actual sensory feedback. Current computational models explaining agency in terms of forward modeling and predictive coding are (...) discussed. Moreover, scientific work has demonstrated that dysfunctional outcome prediction may contribute to aberrant agency experience in schizophrenia and obsessive-compulsive disorder. The chapter concludes with a discussion about the role and nature of internal predictions underlying functional and dysfunctional experience of agency—critically distinguishing the psychological and neural mechanisms operating at different phenomenological levels of agency. Recent work has demonstrated that the sense of agency is not only determined by efference-copy-based internal predictions and internal comparator mechanisms, but by a large variety of different internal and external cues. The study by Moore and colleagues [Moore, J. W., Wegner, D. M., & Haggard, P. . Modulating the sense of agency with external cues. Conscious and Cognition] aimed to provide further evidence for this view by demonstrating that external agency cues might outweigh or even substitute efferent signals to (...) install a basic registration of self-agency. Although the study contains some critical points that, so we argue, are central to a proper interpretation of the data, it hints at a new perspective on agency: optimal cue integration seems to be the key to a robust sense of agency. We here argue that this framework could allow integrating the findings of Moore and colleagues and other recent agency studies into a comprehensive picture of the sense of agency and its pathological disruptions.
