It is a personal matter, a point of autobiography, but it illustrates something that beats in the heart of Cillian McBride’s compact and quietly ambitious book, that I cannot myself choose to value, that I cannot myself choose to esteem, racial or homophobic bigotry. Hence bigots cannot justifiably demand that I recognize the alleged value of their bigotry; nor can they demand such recognition from society more generally, esteem being tied in this way to sincere evaluation. Although a (...) failure to obtain social recognition might result in a loss of self-esteem for some bigots, and although this would in a way be bad, this negative result would be, in another way, simply too bad for bigotry. In this article, I consider the role of institutions in Jacques Derrida’s political engagement. In spite of Derrida’s significant involvement with political causes throughout his life, his engagements have received little sustained attention, and this is particularly true of his work with institutions. I turn to two such cases, the Collège international de philosophie and the Parlement international des écrivains and argue that these represent an alternative mode of institutionalisation. These institutions seek to destabilise other institutions as well as themselves.Looking (...) closely at the institutions that Derrida founded, we see three common characteristics emerge. These institutions are anti-hegemonic,self-reflexive and international. I then connect these to Derrida’s thought, offering a reading of the undecidable, which brings forth the importance of conventions in the decision. Finally, I demonstrate that the three shared characteristics of Derrida’s institutions form part of an effort to open up space for the possibility of alterity. Through this, and beyond a distinction between theory/practice, we come to see Derrida’s institutional engagements as an active form of critique, both of other institutions and themselves. (shrink)

The prospect of vaccinating children and young people (CYP) against COVID raises questions that apply more widely to vaccination in children. When can CYP consent, on their own, for vaccination? What should happen if children and their parents disagree about the desirability of a vaccine? When, if ever, should vaccination proceed despite a child’s dissent or apparent refusal? Background: Critically ill patients often require complex clinical care by highly trained staff within a specialized intensive care unit (ICU) with advanced equipment. There are currently limited data on the costs of critical care in low-and middle-income countries (LMICs). This study aims to investigate the direct-medical costs of key infectious disease (tetanus, sepsis, and dengue) patients admitted to ICU in a hospital in Ho Chi Minh City (HCMC), Vietnam, and explores how the costs and cost drivers can vary between the (...) different diseases. -/- Methods: We calculated the direct medical costs for patients requiring critical care for tetanus, dengue and sepsis. Costing data (stratified into different cost categories) were extracted from the bills of patients hospitalized to the adult ICU with a dengue, sepsis and tetanus diagnosis that were enrolled in three studies conducted at the Hospital for Tropical Diseases in HCMC from January 2017 to December 2019. The costs were considered from the health sector perspective. The total sample size in this study was 342 patients. -/- Results: ICU care was associated with significant direct medical costs. For patients that did not require mechanical ventilation, the median total ICU cost per patient varied between US$64.40 and US$675 for the different diseases. The costs were higher for patients that required mechanical ventilation, with the median total ICU cost per patient for the different diseases varying between US$2,590 and US$4,250. The main cost drivers varied according to disease and associated severity. -/- Conclusion: This study demonstrates the notable cost of ICU care in Vietnam and in similar LMIC settings. Future studies are needed to further evaluate the costs and economic burden incurred by ICU patients. The philosophical world has long attempted to reconcile determinism with the notion of free will. Everything we have observed above the sub-atomic level appears to operate in a strictly causal relationship with other objects. This appears to indicate that all events are completely causally determined. On the other hand, we each clearly sense that we are more than mere causal automatons. We make choices, and we exercise free will, and these choices affect our future. Thus, the notions of determinism and (...) free will are at odds. Over the course of the last four decades, William Leon McBride has distinguished himself as one of the most esteemed and accomplished philosophers of his generation. This volume—which celebrates the occasion of his seventy-fifth birthday—includes contributions from colleagues, friends, and formers students and pays tribute to McBride’s considerable achievements as a teacher, mentor, and scholar.

The concept of ‘Ethnicity’ still enjoys some currency in the historical and social science literature. However, the cogency of the idea remains disputed. First coming to prominence in the 1980s, the word is often used to depict the character of social relations in the context of conflicts over sovereignty. The case of Northern Ireland presents a paradigmatic example. This article is a rejoinder to Ian McBride’s contention that my scepticism about the notion lacks justification. With reference to disputes over (...) the state, I show in response that ‘ethnicity’ in effect means nationality. I further claim that the nation state is a successor to the dynastic state. In clarifying the meaning of this arrangement, the article brings out how the nation is a juridical rather than empirical category. More specifically, it derives from the notion of corporate personality in law. For this reason, its retrospective integrity is a matter of fabrication, depending on the fiction of ancestral continuity. At the same time, its future-oriented cohesiveness means that it must be invested with a unifying will. I conclude that the legitimacy of a nation state rests on its democratic will, whose coherence is expressed in the action of its government. (shrink)
