Objective: to evaluate CANLPH score and systemic immune inflammation index (SII) in patients with symptomatic rheumatic mitral stenosis (MS) undergoing percutaneous mitral balloon valvuloplasty (PMBV). -/- Methods: 62 patients who underwent PMBV in our clinic between 2018 and 2021 were included retrospectively. The patients were divided into 2 groups according to echo score. The CANLPH score was calculated from the cut-off values of C-reactive protein to albumin ratio (CAR), neutrophil to lymphocyte ratio (NLR) and platelet to hemoglobin (...) ratio (PHR), determined with the Youden index and SII by the formula platelet x neutrophil/lymphocyte. P <0.05 was considered statistically significant. -/- Results: The mean age of the patients was 44.5±10.4 years (40 female, 64.5%). The mean values of SII and CANLPH scores were higher in the Echo score >8 group (p <0.001, both). The mean mitral gradient before and after PMBV was 12.6±5.7mm Hg and 5.0±2.4 mm Hg, and the mean valve area was 1.12±0.27 cm2 and 1.85±0.29 cm2. A statistically significant and negative correlation was observed between the gradient decrease after the procedure and the CANLPH score and SII (r=-0.426, p=0.001 and r=-0.418, p=0.001, respectively). In receiver operating characteristic (ROC) curve analysis, it was concluded that the ability of CANPLH score to predict the higher Echo scores was noninferior to SII with an area under curve (AUC:0.820 (0.701-0.906) and AUC:0.786 (0.664-0.880), z statistics 0.576 and p=0.564. -/- Conclusion: SII and CANLPH scores are correlated with Echo score. A significant negative relationship was found with both biomarkers and gradient decrease after the PMBV. (shrink)

Our examination of universal origins and fine-tuning will begin with a discussion of infl ationary scenarios grafted onto Big Bang cosmology and the proof that all infl ationary spacetimes are past-incomplete. After diverting into a lengthy critical examination of the “different physics” offered by quantum cosmologists at the past-boundary of the universe, we will proceed to dissect the inadequacies of infl ationary explanations and string-theoretic constructs in the context of three cosmological models that have received much attention: the Steinhardt-Turok cyclic (...) ekpyrotic model (which does not invoke inflation), the Gasperini-Veneziano pre-Big Bang infl ationary model, and the inflationary string landscape model advanced by Susskind, Polchinski, Bousso, and Linde. We will argue that none of these highly speculative string cosmologies removes the necessity of a beginning to the process of universe generation, and we will emphasize the implications of this fact. Then, since the inflationary “mechanism” only really addresses the fi ne-tuning of the initial conditions of the universe and not the conditions embodied in its finely tuned laws and constants, we will analyze the adequacy of the string multiverse in its three versions (cyclic ekpyrotic, pre-Big Bang, and landscape) for explaining the nomological structure and values of these precisely tuned life-compatible universal parameters. When all is said and done, it will be clear that transcendent intelligent agency is not just the only causally suffi cient and therefore metaphysically sound explanation for universal origins and fine-tuning, but it is also much more parsimonious, elegant, and resonant with meaning than all of the ad hoc machinations of multiverse cosmology. Humanity may underestimate the rate of natural global catastrophes because of the survival bias (“anthropic shadow”). But the resulting reduction of the Earth’s future habitability duration is not very large in most plausible cases (1-2 orders of magnitude) and thus it looks like we still have at least millions of years. However, anthropic shadow implies anthropic fragility: we are more likely to live in a world where a sterilizing catastrophe is long overdue and could be triggered by unexpectedly small human (...) actions. In the same way, an over-inflated toy balloon, which will soon burst, is very fragile. Anthropic fragility can manifest itself in the higher chances of runaway global warming. It has often been suggested that the Earth's atmosphere remained life-supporting for billions of years by sheer chance. Therefore, the survival bias can be strong. It is also known that Earth-like water worlds could experience transitions into deadly moisture greenhouse (mean T = 65C). This means that relatively small anthropogenic actions could put the climate above an unpredictable tipping point, which could lead to the moisture greenhouse. Thus, it is necessary to carry out urgent geoengineering studies and prepare to prevent an unexpected climate catastrophe. There are three main counterarguments against the existence of the anthropic shadow: self-indication assumption (SIA), past observers and the Gaia hypothesis; we show that they fail. It was proposed that SIA exactly compensates the anthropic shadow as an observer unlikely to find herself in a world with a strong anthropic shadow; however, there is a baseline level of the anthropic shadow for all habitable planets, similar to the rate of evolutionary transitions like abiogenesis. There are no “past observers” as qualified observers appeared only 50 years ago. Gaia hypothesis assumes existence of self-stabilizing feedback in climate, but new types of events like quick CO2 growth could override its coping ability. We present a list of other catastrophes that may have been underestimated because of the anthropic shadow, including collider catastrophes, nuclear war and even an alien invasion. We also hypothesized that human intelligence is more likely to emerge in an unstable world that is nearing its end and thus we get a new form of Doomsday argument. (shrink)

The essay offers a thought experiment to try to clarify our distinction between our naïve ancestors and our sophisticated moderns. The effect of the thought experiment is to cast doubt upon the distinction and examine further our own myths about our ancestors. And to wonder at what it means to be truly modern.

Semantics traditionally focuses on linguistic meaning. In recent years, the Super Linguistics movement has tried to broaden the scope of inquiry in various directions, including an extension of semantics to talk about the meaning of pictures. There are close similarities between the interpretation of language and of pictures. Most fundamentally, pictures, like utterances, can be either true or false of a given state of affairs, and hence both express propositions (Zimmermann, 2016; Greenberg, 2013; Abusch, 2015). Moreover, sequences of pictures, like (...) sequences of utterances, can be used to tell stories. Wordless picture books, comics, and film are cases in point. In this paper I pick up the project of providing a dynamic semantic account of pictorial story-telling, started by Abusch (2012) and continued by Abusch & Rooth (2017); Maier & Bimpikou (2019); Fernando (2020). More specifically, I propose here a semantics of speech and thought bubbles by adding event reference to PicDRT. To get there I first review the projection-based semantics for pictures (section 1), noting the fundamental distinction between symbolic and iconic meaning that makes speech bubbles especially interesting (section 2). I then review the dynamic PicDRT framework for pictorial narratives (section 3), add events (section 4), and propose an account of speech bubbles as quotational event modification (section 5). I end with a brief look at other conventional, symbolic enrichments in comics (section 6). (shrink)

Against this background, the critique of democracy becomes not only possible but also popular, and examples in China and many Western democracies abound. It is no exaggeration to say, in this context, that sufficient momentum has gathered to qualify the situation as "democratic recession," despite people may have different understandings as to the exact (...) nature of this recession and how long it will last. Accompanying this recession, Confucian political meritocracy, whose aim is to correct the wrongs of democracy, is constructed as an alternative to liberal democracy. Following the previous relevant works [1]-[3], a scaling analysis is performed to derive a set of scaling criteria which were thought to be suitable for reproducing the major thermal-hydraulic phenomena in a scaled-down CANDU moderator tank similar to that in a prototype power plant during a full power steady state condition. The objective of building a scaled-down moderator tank is to generate the experimental data necessary to validate the computer codes which are used to analyze the accident analysis of CANDU-6 (...) plants. The major variables of interests in this paper are moderator flow velocity and temperature of the moderator which is D2O inside the moderator tank during a steady state and transient conditions. The reason is that the local subcooling of the moderator is found to be a critical parameter determining whether the stable film boiling can sustain on the outer surface of the calandria tube if the contact of overheated pressure tube and cold calandria tube should occur due to pressure tube ballooning during LBLOCA with ECC injection failure [4]. The key phenomena involved include the inlet jet development and impingement, buoyancy force driven by the moderator temperature gradient caused by non-uniform direct heating of the moderator, and the pressure drop due to viscous friction of the flow across the calandria tube array. In this paper, the previous researches are reviewed, some concerns or potential problems associated with them implied by comparing CFD analyses results between the CANDU-6 moderator tank and 1/4 scaled-down test facility are described, and as a way to examine the assumption of the scaling analysis is true an order-of-magnitude analyses are performed. Based on the results of these analyses the assumption of neglecting and terms cannot be justified for the power of 0.5 MW and 1.566 MW for the 1/4 scaled-down facility. Further investigation is thought to be necessary to confirm this result, i.e. if the scaling of the previous work1 is justifiable by some other independent analyses. (shrink)

Stove's article, 'So you think you are a Darwinian?'[ 1] was essentially an advertisement for his book, Darwinian Fairytales.[ 2] The central argument of the book is that Darwin's theory, in both Darwin's and recent sociobiological versions, asserts many things about the human and other species that are known to be false, but protects itself from refutation by its logical complexity. A great number of ad hoc devices, he claims, are used to protect the theory. If co operation is observed (...) where the theory predicts competition, then competition is referred to the time of the cavemen, or is reinterpreted as competition between some hidden entities like genes or abstract entities like populations. In a characteristic sally, Stove writes of the sociobiologists' oscillation on the meaning of kin altruism: Any discussion of altruism with an inclusive fitness theorist is, in fact, exactly like dealing with a pair of balloons connected by a tube, one balloon being the belief that kin altruism is an illusion, the other being the belief that kin altruism is caused by shared genes. If a critic puts pressure on the illusion balloon - perhaps by ridiculing the selfish theory of human nature - air is forced into the causal balloon. There is then an increased production of earnest causal explanations of why we love our children, why hymenopteran workers look after their sisters, etc., etc. Then, if the critic puts pressure on the causal balloon - perhaps about the weakness of sibling altruism compared with parental, or the absence of sibling altruism in bacteria - then the illusion balloon is forced to expand. There will now be an increased production of cynical scurrilities about parents manipulating their babies for their own advantage, and vice versa, and in general, about the Hobbesian bad times that are had by all. In this way critical pressure, applied to the theory of inclusive fitness at one point, can always be easily absorbed at another point, and the theory as a whole is never endangered.[ 3] Now, it is uncontroversial to assert that Darwinism is a logically complex theory, and that its relation to empirical evidence is distant and multi faceted. One does not directly observe chance genetic variations leading to the development of new species, or even continuous variations in the fossil record, but must rely on subtle arguments to the best explanation, scaling up from varieties to species, and so on..
