MedDRA (the Medical Dictionary for Regulatory Activities Terminology) is a controlled vocabulary widely used as a medical coding scheme. However, MedDRA’s characterization of its structural hierarchy exhibits some confusing and paradoxical features. The goal of this paper is to examine these features, determine whether there is a coherent view of the MedDRA hierarchy that emerges, and explore what lessons are to be learned from this for using MedDRA and similar terminologies in a broad medical informatics context that includes relations among (...) multiple disparate terminologies, thesauri, and ontologies. We performed a qualitative analysis of the Thesaurus in order to assess its conformity with principles of good practice in terminology and ontology design. We used both the on-line browsable version of the Thesaurus and its OWL-representation (version 04.08b, released on August 2, 2004), measuring each in light of the requirements put forward in relevant ISO terminology standards and in light of ontological principles advanced in the recent literature. Version 04.08b of the NCI Thesaurus suffers from the same broad range (...) of problems that have been observed in other biomedical terminologies. For its further development, we recommend the use of a more principled approach that allows the Thesaurus to be tested not just for internal consistency but also for its degree of correspondence to that part of reality which it is designed to represent. The present essay is devoted to the application of ontology in support of research in the natural sciences. It defends the thesis that ontologies developed for such purposes should be understood as having as their subject matter, not concepts, but rather the universals and particulars which exist in reality and are captured in scientific laws. We outline the benefits of a view along these lines by showing how it yields rigorous formal definitions of the foundational relations used in many influential (...) ontologies, illustrating our results by reference to examples drawn from the domain of the life sciences. It is only by fixing on agreed meanings of terms in biomedical terminologies that we will be in a position to achieve that accumulation and integration of knowledge that is indispensable to progress at the frontiers of biomedicine. Standardly, the goal of fixing meanings is seen as being realized through the alignment of terms on what are called ‘concepts’. Part I addresses three versions of the concept-based approach – by Cimino, by Wüster, and by Campbell and associates – and surveys (...) some of the problems to which they give rise, all of which have to do with a failure to anchor the terms in terminologies to corresponding referents in reality. Part II outlines a new, realist solution to this anchorage problem, which sees terminology construction as being motivated by the goal of alignment not on concepts but on the universals (kinds, types) in reality and thereby also on the corresponding instances (individuals, tokens). We outline the realist approach, and show how on its basis we can provide a benchmark of correctness for terminologies which will at the same time allow a new type of integration of terminologies and electronic health records. We conclude by outlining ways in which the framework thus defined might be exploited for purposes of diagnostic decision-support. (shrink)

In this paper, the author develops a theory of concepts and shows that it captures many of the ideas about concepts that Leibniz expressed in his work. Concepts are first analyzed in terms of a precise background theory of abstract objects, and once concept summation and concept containment are defined, the axioms and theorems of Leibniz's calculus of concepts (in his logical papers) are derived. This analysis of concepts is then seamlessly connected with Leibniz's modal metaphysics of complete individual concepts. (...) The fundamental theorem of Leibniz's modal metaphysics of concepts is proved, namely, whenever an object x has F contingently, then (i) the individual concept of x contains the concept F and (ii) there is a (counterpart) complete individual concept y which doesn't contain the concept F and which `appears' at some other possible world. Three different notions of concepts are outlined: one derives from Leibniz, while the other two derive from Frege. The Leibnizian notion is the subject of his "calculus of concepts" (which is really an algebra). One notion of concept from Frege is what we would call a "property", so that when Frege says "x falls under the concept F", we would say "x instantiates F" or "x exemplifies F". The other notion of concept from Frege is that of the notion of (...) sense, which played various roles within Frege's theory. This notion of concept can be generalized and, as such, accounts for our intuitive talk of "x's concept of ...", where the ellipsis can be filled in with a name for individual, a property, or a relation, etc. After outlining these three notions, I then discuss how (axiomatic) object theory offers a distinct, precise regimentation of each of the three notions.
