    In this paper, I respond to the following argument which several authors have presented. If we are culpable for some action, we act either from akrasia or from culpable ignorance. However, akrasia is highly exceptional and it turns out that tracing culpable ignorance leads to a vicious regress. Hence, we are hardly ever culpable for our actions.     We respond to a Letter to the Editor regarding "Principlism and contemporary ethical considerations for providers of transgender health care." We address criticisms by Castle & Klein (2024) of blatant fatphobia related to the ethical elements concerning BMI restrictions for gender-affirming surgery. Our response corrects several mischaracterizations of the article and clarifies our position. My co-authors and I remain focused on advocating for patient-centered, ethically sound, evidence-based, and equitable healthcare policies.

    This article discusses Rik Peels's response to Williams's argument against voluntary belief. Williams argues that voluntary beliefs must be acquired independently of truth-considerations, so they cannot count as beliefs after all, since beliefs aim at truth. Peels attempted to reply by showing that in cases of self-fulfilling beliefs, a belief can indeed be voluntarily acquired in conditions which retain the necessary truth-orientation.
