Summary Giancarlo Trombini presents the continuation of his research on the question of which criteria can be used to assess the progress of therapy in an objectively verifiable way and to make the decision on the completion of therapy. In the first phase of his research, the phenomenological criterion of a qualitative change in the patient’s relations toward the positive and higher complexity was proposed for this purpose. In terms of the working method in analytic therapy, (...) this meant concretely: attention should be paid to what development is shown in the comparison of the relationships that occur in the dream narrative and in the subsequent associations. This criterion was therefore given the name manifest dream/association comparison (MDAC)—comparison between the manifest dream and the subsequent associations. The idea can easily be transferred to those therapy methods, which do not primarily work with reports of dream memories and subsequent associations—also, in other ways of working, it is possible to pay attention, in the way suggested by Trombini, to the qualitative development of the relationships which are thematized by the clients in the course of an hour. To this first criterion, another phenomenological criterion is now added in the present article: that of the “concluding therapeutic turn” (CTT). If the patient’s development reaches this turn in the course of the therapy in one session, this indicates, according to Trombini, that the therapy can soon be concluded. The fulfillment of this criterion can be recognized by the fact that in the sequence of dream narration and subsequent associations in a session, a relational dynamic toward the positive and higher complexity becomes recognizable and that is, at the same time, connected with a reconciliation of the three temporal reference systems (past, present, and future). The achievement of this CTT indicates that the patient is aware of the changes made in therapy and makes it evident to the therapist that the therapy is nearing completion. (shrink)

Therapists and related theorists and practitioners of mental health tend to hold one of two broad views about how to help patients. On the one hand, some maintain that, or at least act as though, the basic point of therapy is to help patients become clear about what they want deep down and to enable them to achieve it by overcoming mental blockages. On the other hand, there are those who contend that the aim of therapy should instead (...) be to psychologically enable patients to live objectively desirable lives, say, ones that involve developing their inherent talents or exhibiting an authentic/strong/integrated self. In this chapter, I argue that neither of these prominent approaches is complete. Contra the former, sometimes what patients want deep down is not something to promote, and, against the latter, it can sometimes be reasonable for patients to want to sacrifice their own objective interests and for therapists to assist them in this regard. I argue that the category of meaningfulness does well at accounting for these counterexamples to the two dominant approaches, while capturing the kernels of truth in them. The proper aim of therapy, I suggest, is to enable patients to live meaningfully. (shrink)

Background: Multi Drug Therapy approved by WHO is the best treatment option for Leprosy. There is a significant decline of mortality and morbidity after the introduction of multi drug therapy. But the adverse effects causing changes in clinical and laboratory parameters to multi drug therapy are the main limiting obstacle for the treatment course completion. Objective: The aim of this study is to find out the effects on routine laboratory parameters including hematological and biochemical changes before, (...) during and after the treatment of multi drug therapy in leprosy. Methodology: A descriptive cohort hospital based study on 85 multi drug therapy treated leprosy patients using standard laboratory tools for routine investigations for 18 months. Results: During the one year multidrug treatment period, most of the routine laboratory parameters showed mild to moderate change from normal level during the first 3 months with gradual recovery in the following months. Complete blood count assessment revealed mild decline in Hb, PCV, MCH, MCHC, and moderate decline in WBC and Platelet count. A mild increase in liver function tests (Bilirubin, AST, ALT, Alkaline phosphatase) and renal function tests (S.Creatinine, b. Urea) was observed. A moderate decline in S. Sodium with mild increase in S. Potassium was found on investigating S. Electrolytes. Patients were fully recovered from all hemato-biochemical adverse effects after 6 months of completion of the multi drug therapy. Conclusion: Despite of mild to moderate hematological and biochemical adverse reactions with multi drug therapy, recovery was excellent after 6 months of completion of therapy. (shrink)

It has been known for long time that the cosmic sound wave was there since the early epoch of the Universe. Signatures of its existence are abound. However, such an acoustic model of cosmology is rarely developed fully into a complete framework from the notion of space, cancer therapy up to the sky. This paper may be the first attempt towards such a complete description of the Universe based on classical wave equation of sound. It is argued that one (...) can arrive at a consistent description of space, elementary particles, Sachs-Wolfe acoustic theorem, up to a novel approach for cancer therapy, starting from this simple classical wave equation of sound. We also discuss a plausible extension of Acoustic Sachs-Wolfe theorem based on its analogue with Klein-Gordon equation to become Acoustic Sachs-Wolfe-Christianto-Smarandache-Umniyati (ASWoCSU) equation. It is our hope that the new proposed equation can be verified with observation data. But we admit that our model is still in its infancy, more researches are needed to fill all the missing details. Objective: Patients' satisfaction is one of the most important goals in complete denture therapy, and there are many factors influencing this parameter. This study aimed to determine patient satisfaction with conventional removable complete denture made by clinical year students at the Faculty of Dentistry, Al Azhar University - Gaza. Methods: A sample of 85 patient who had conventional removable complete denture made by clinical year students at the Faculty of Dentistry Al Azhar University - Gaza filled a questionnaire two (...) months after denture insertion. The questionnaire involving four sections including personal information, history, denture evaluation in aspects of mastication, speech, aesthetics and retention, and patient comfort. Results: Of the 85 patient studied, 81% were males, and the mean age was 60.3 years SD+-9.64. The majority (n= 67, 79 %) lost their teeth because of bad oral hygiene. The overall satisfaction level was (92.1%), and patients were more satisfied with their upper denture. A significant association was found between the patient satisfaction and prior denture use experience (p=0.013). No significant correlation was established between the patient satisfaction and their gender (p=0.188), age group (p=0.640) and employment status (p=0.667).Conclusion: The patients have shown a high level of overall satisfaction. A significant association was found between the overall satisfaction level of patients and prior denture use experience. No statistically significant association was observed between the overall satisfaction level of patients and with each of age group, employment status and gender. (shrink)

5-MeO-DMT is a psychedelic substance with a short duration of action and intensive effects. Its therapeutic efficacy and practicality may significantly surpass those of classical psychedelics such as ayahuasca and LSD. -/- This retrospective ethnographic inquiry features a woman in her mid-thirties who witnessed her mother's violent suicide and its bloody aftermath at the age of three. Before and after that, her childhood was characterized by domestic violence and sexual abuse perpetrated by several members of her family and extended family. (...) In her twenties and thirties, she dated a member of the local mafia with the intention of asking him to kill her father, who had been the main perpetrator of the sexual abuse and violence. This plan was eventually not carried out, but it reflected her deep bitterness and wrath. -/- A process initiated in her early thirties involving four 5-MeO-DMT sessions and a few additional sessions with psilocybin and ayahuasca in the course of two years completely resolved her symptoms related to the abuses, to the extent that she could rebuild a functional relationship with her father and feel love and compassion towards him. This outcome, i.e., the complete reversal of her attitude and emotions towards her father, appeared highly unusual. For the last three years, the outcome had remained stable. -/- The article also presents the perspective of a female facilitator of this treatment process. The article contributes to a better understanding of the use of 5-MeO-DMT in severe traumatization as well as exemplifies the possible positive contributions of actors who are not medical professionals in resolving deep collective traumatization in societies. One of the leading exponents of W's ideas on the language games of inner and outer (the `Two Selves' operation of our personality or intentionality or EP etc.) is the prolific Daniel Hutto (DH). His approach is called `Radical Enactivism' and is well explained in numerous recent books and papers (see my review of Radicalizing Enactivism) and a new one is appearing as I write (Evolving Enactivism). It is a development of or version of the Embodied Mind ideas now current (...) and, cleansed of its jargon, it is a straightforward extension of W's 2nd and 3rd period writings (though Hutto seems only intermittently aware of this). -/- Unfortunately, in 2006 Hutto had not yet arrived at his Radical Enactivism, so much time is wasted on McDowell and Brandom and of course none of them to this day have totally digested the later W and his prescient analysis of automatic behavior and the two systems of thought - so fully in tune with contemporary research. Nor is there any discussion of Searle's groundbreaking and completely Wittgensteinian (unwittingly) disquisitions on the Construction of Social Reality. Thus, his chapters 5 and 6 on Realism and Idealism etc., though superb for 2002, need a complete rewrite from a modern two systems viewpoint and I provide a start on that in my review. Much time is wasted on Davidson and Williams, etc. but one can endure them for Hutto's brilliant analyses and the frequent quotes from W. The last chapter gives his critic Rupert Read the counterblast he deserves and permits a slight update to 2006. There is currently no generally agreed-upon definition of the mechanism of action of psychedelic therapy. Existing proposals have approached the issue from various perspectives, utilizing concepts on many layers of abstraction. Most commonly, mechanisms based on neurotransmitters have been proposed. From a clinical perspective, explanations on the psychological level would be more useful. This study provides one such explanation, focusing on the destabilization of trauma-related memories and their replacement with memories that allow for more adaptive behaviors. This mechanism is (...) not specific to psychedelics, and the study therefore illustrates a non-pharmacological process involving it. -/- The study features a male entrepreneur in his early fifties who suffered from chronic but non-debilitating anxiety and depression since his teenage years. In his mid-30s, he suffered from severe burnout but partially recovered. The amount of alcohol consumed was constantly relatively high. More recently, due to simultaneous relationship and workplace stress, he experienced a more severe episode of burnout, depression, anxiety, and insomnia. He felt completely exhausted when trying to get something done and became fearful of the workplace. Psychotherapy and various pharmaceutical medications provided little benefit. -/- After three months on sick leave, a trip to the mountains triggered severe insomnia. Sleep deprivation likely contributed to a spontaneous experience of altered states of mind, including a two-day period of hallucinatory visions. There was the feeling of being a complete failure, but through a new relationship initiated just before the trip, he received love and validation from his new partner, which contradicted the feeling of worthlessness. The combination of an altered state of mind and love induced an alteration of identity that resolved his depression, burnout, and sleep disorders. After three more months of self-organized rehabilitation, he successfully returned to work. -/- Six months after the trip, financial decline caused by his absence and unchanged stressful conditions at the workplace led to disappointment, motivational issues, and stress, but his work performance remained good regardless. One year after the trip, although his work-related stress had increased rather than decreased due to the financial situation of his company being more demanding than before his burnout, the positive outcome prevailed, proving its long-term stability. This is an early volume from a much respected psychedelic psychotherapist. He has written several other books since this one but until recently none of his books were on Amazon and still you can only find a German edition and a Spanish one (from 1993) but no English one (except a couple used copies). This is sad since these drugs have enormous therapeutic potential but afaik government suppression still prevents their use. The most interesting and readable parts are the case (...) histories of drug therapy, especially his own-e.g., on pages 114, 166 and 178. Though a very advanced therapist compared to many, he is far from finished with his intellectual growing up and clings to his psychoanalytic background even though he often mentions its extreme shortcomings. The often painfully awkward text is full of insights from his own therapeutic voyages and those of his patients interspersed with psychoanalytic nonsense. Part of the awkwardness may be due to bad translation from the German but the lack of a rational spiritual framework is a major problem (as with most therapy-or most life for that matter) that he only occasionally seems to recognize. He also sometimes veers off into the ozone-e.g., after many pages of sensible dialog he can opine that varicose veins and cancer are a result of the splitting of a psychosis (p99)! He quotes Zen and Sufi stories, Krishnamurti and Al Ghazali and even Castenada (seemingly unaware of his exposure as a fraud 20 years ago!), yet he seems oblivious to the fact that meditation is the most powerful therapy there is and to the presence around him in his own country and all over the globe of the most diverse and effective therapeutic community that has ever existed-that of the students of the great Indian mystic (ie, psychotherapist) Osho. What a great pity! He could have learned so much and advanced his therapy so far. But like most people he deliberately or unconsciously avoids anything which might wake him up too much. It's possible however that he just avoids mentioning Osho as he's radical enough to trigger the control templates in the monkey brain and enlightenment has the power to change one's whole life and not just put a bandaid on as other therapies usually do. Widmer refers repeatedly to the great LSD therapy pioneer Stanislav Grof, whose writings are one of the first that the interested reader may want to consult. Also, anyone who has a serious interest in psychedelic therapy should read Myron Stolaroff´s " The Secret Chief "-an account of the most remarkable clandestine therapist of this type of all time. It tells the story of the late Leo Zeff, who helped thousands to find themselves with the skilled and varied use of a wide variety of psychedelics. As the book is hard to find you may wish to read the info on Zeff on the web. In addition the clinical chapters in the excellent Ecstasy:the complete guide by J. Holland(2001) and her web page www.drholland.com (and elsewhere on the net) provide a broad framework and guide to therapy that is lacking here. Body dysmorphic disorder (BDD) poses a significant challenge to an individual’s mental well-being. The obsessive preoccupation with perceived defects in one’s appearance affects individuals’ daily functioning and can result in serious risks, including suicidal ideation and self-surgery. While treatments such as cognitive behavioural therapy and serotonin reuptake inhibitors can provide relief, they do not achieve complete remission. It has been suggested that therapy should not only interrupt the harmful behaviour, but should also address their sense of self, and (...) that after addressing their internalised values, individuals with BDD will be able to incorporate their perceived defect in a manner similar to how individuals with real deformities often do. Phenomenology of psychopathology analyses subjectivity in cases of pathology, and analyses of BDD show that the perceived defect has become a locus of shame. This implies that individuals with BDD have permanently incorporated the gaze of the Other and constantly experience their body as a body-for-others. This paper further explores the sense of self in individuals with BDD through Jenny Slatman’s interpretation of bodily integrity: bodily integrity as a never-ending process of identification. By comparing the experiences of individuals with BDD to those with real deformities and to those who have undergone cosmetic surgery, I will show that re-identification does not occur in individuals with BDD. This suggests that their bodily integrity is fundamentally disturbed and that addressing their internalised values alone will probably not be sufficient for the disorder to go into remission. In this chapter, I describe my “post-diagnosis” experiences as the parent of an autistic child, those years in which I tried, but failed, to make sense of the overwhelming and often nonsensical information I received about autism. I argue that immediately after being given an autism diagnosis, parents are pressured into making what amounts to a life-long commitment to a therapy program that (they are told) will not only dramatically change their child, but their family’s financial situation and even (...) their entire mode of existence. Moreover, despite information overload, many treatment programs for autism rely on empty jargon and make completely unrealistic promises, so parents are left feeling overwhelmed and panicked. Even well respected therapy programs encourage parents to spend liberally. Indeed, autistic therapists, who help construct what I refer to as the Culture of Autism, advise parents to commit to a minimum of 35 to 45 hours of intensive therapy every week. The implications are clear: for a parent who works full-time, their autistic child becomes a second full-time job. Autism is big business right now, and therapists are pushing parents to the brink of desperation. So it is not too surprising that there is a desperate cry for a more permanent solution—which is why researchers seek to cure autism. But there are two ways to conceptualize cure. A Therapeutic Cure model (TC) conceives of a cure as a beneficial treatment for the patient that eliminates or ameliorates the harms of the disease or condition. But the notion of a therapeutic cure for autism is highly implausible, given the complexities of autism. Indeed, at this point, the vast majority of researchers have come to the conclusion that the idea of a therapeutic cure for autism is simply a non-starter. Therefore the bulk of research seeking a cure for autism focuses instead on a second approach, which I refer to as the Negative Eugenics Cure model (NEC). With this model, the intention is to eliminate the disease or condition without regard for the health or well-being of the organism carrying the disease or condition. So, with regard to autism, researchers are focusing on identifying genetic markers for autism that can be detected in utero, or in embryos, so that autistic fetuses can be eliminated and autism eradicated by preventing the existence of autistic individuals. I review both models and argue that both fail to provide convincing arguments that the “solution” either offers is desirable. Both rest on the assumption that autism renders a life not worth living which, all things considered, is false. Instead of pushing to cure autism, an idea pervasive in this Culture of Autism, I contend that autistics are individuals with lives worth living. Moreover, rather than expend millions on research to search for the means to eliminate autism, we should instead expend our resources to ensure autistic individuals have access to support they may need. If the phenomenology of autism were better understood and appreciated, the panicked demand for a cure for autism might abate and perhaps autism could be seen as having value in and of its own right. In an online, participatory class, we interpreted The Dream of the White House knowing nothing of the dreamer and having none of the dreamer’s associations. Our interpretation included a series of falsifiable predictions about the dreamer. When it was complete, we asked the bringer of the dream (who had until then been silent and was not visible to us) to give us more information about the dreamer. Of 17 predictions 15 were confirmed. The dreamer suffers dislocation and loss until she (...) asks a female manager for help. This record is another iteration of an experiment that will be described more fully in the paper 'The Dream of the Six-Legged Dog: An Experimental System to Test Meaning,' soon to be published. This iteration repeats and confirms the evidence given in that paper. More than any of the other iterations, it details how talk therapy promotes personality reorganization. It then supports these observations with experimental evidence. Because it demonstrates how talk therapy works, this iteration supports talk therapy against pressure from the drug and insurance corporations. Purpose Prescription stimulant use as “cognitive enhancers” has been described among undergraduate college students. However, the use of prescription stimulants among future health care professionals is not well characterized. This study was designed to determine the prevalence of prescription stimulant misuse among students at an academic health sciences center. -/- Method Electronic surveys were e-mailed to 621 medical, pharmacy, and respiratory therapy students at East Tennessee State University for four consecutive weeks in fall 2011. Completing the survey was voluntary (...) and anonymous. Surveys asked about reasons for, frequency of, and side effects of nonprescription misuse of prescription stimulants. Given the sensitive material, an opportunity to win one of ten $50 gift cards was used as an incentive. -/- Results Three hundred seventy-two (59.9%) students completed the survey from three disciplines (47.6% medical, 70.5% pharmacy, and 57.6% respiratory therapy). Overall, 11.3% of responders admitted to misusing prescription stimulants. There was more misuse by respiratory therapy students, although this was not statistically significant (10.9% medicine, 9.7% pharmacy, 26.3% respiratory therapy; P = .087). Reasons for prescription stimulant misuse included to enhance alertness/ energy (65.9%), to improve academic performance (56.7%), to experiment (18.2%), and to use recreationally/get high (4.5%). -/- Conclusions Prescription stimulant misuse was prevalent among participating students, but further research is needed to describe prevalence among future health care workers more generally. The implications and consequences of such misuse require further study across professions with emphasis on investigating issues of academic dishonesty (e.g., “cognitive enhancement”), educational quality, and patient safety or health care quality. The essay consists in the analysis of the problem of the evil in the man and in the analysis of the remedies which the man can find against the evil. Plato affirms the presence of an active principle of evil in the soul of every man, which coincides with some instincts of the appetitive soul; the opposite principle to the evil is the reason, which needs, though, a correct education in order to be able to fight efficiently against the evil (...) in us. The man can be seen as a battle field of these opposite forces. Plato describes the presence of the evil in us in some passages of Republic Book 9, where he compares the appetitive part of the soul with a monster. The destiny of every person in her earthly existence consists in the continuing control of the appetitive part of the soul, if the status of ethical education is to be reached and maintained. The man who remains in the realm of the opinion, that is, in the realm of the doxa is an individual who only disposes of unstable opinions and who as a consequence do not have authentic remedies against the appetitive part. On the contrary, the individual who can ascend to the realm of being through the hard education represented by arithmetic, geometry, stereometry, astronomy, harmony and, finally, dialectic is really able to contrast the force of the evil within the individual. Ethics is really possible only through the complete education which passes through these disciplines: the more the individuals is theoretically educated, the more the individual is ethically educated. The knowledge of ideas is the only authentic therapy against the evil in us. (shrink)

INTRODUÇÃO O anestro pós-parto é o período após o parto no qual a fêmea não apresenta ciclos estrais (atividade cíclica). Na vaca leiteira, o parto é seguido de um período de inatividade ovariana de duração variável, que é principalmente afetada pelo estado nutricional, produção leiteira, ganho ou perda de condição corporal antes e depois do parto, e por condições patológicas como hipoplasia dos ovários, cistos ovarianos, mumificação uterina, piometra entre outras, além, também, de condições ambientais como instalações que podem causar (...) estresse etc. 1. Controle neuroendócrino do anestro Entre os 12 e 15 dias pós-parto, as células gonadotróficas presentes na adenohipófise começam a produzir e secretar o FSH (hormônio folículo estimulante), o qual estimula a primeira onda folicular; entretanto, nenhum folículo dominante do(s) ovário(s) chega a completar seu desenvolvimento devido à carência de estímulo apropriado do LH (hormônio luteinizante) que também é produzido pelas mesmas células e que deve estar presente em altas concentrações. Nas 2 primeiras semanas pós-parto (primeiros 15 dias), as concentrações circulantes de LH são baixas, o que se deve à ausência de reservas do LH e à incapacidade temporal da hipófise para responder ao estímulo da GnRH (hormônio liberador de gonadotrofina). Entre os dias 15 e 20 pós-parto, a hipófise aumenta a sua capacidade para responder ao GnRH; entretanto, o estabelecimento do padrão de secreção do LH adequado para estimular a maturação e a ovulação do folículo dominante das primeiras ondas foliculares depende fundamentalmente do balanço energético. 2. Fatores que determinam o início da atividade ovariana pós-parto Na vaca leiteira, é frequente que algum dos primeiros folículos dominantes que se desenvolvem durante as primeiras duas ou três semanas complete a sua maturação e chegue à ovulação, isto é, madure e ovule. O fator limitante para o reinício da atividade ovariana é o balanço energético negativo (BEN). No gado leiteiro, o consumo de matéria seca (CMS), após o parto, deverá ser aumentado em até quatro vezes, para cobrir a demanda de nutrientes para a produção de leite e mantença; no entanto, a vaca é incapaz de consumir a exigência de matéria seca, por isso recorre a suas reservas de gordura e proteína, e cai em BEN. As vacas que consomem menos matéria seca, produzem menos leite, têm um BEN mais profundo e o período do parto à primeira ovulação é maior. O nível mais baixo do BEN é alcançado entre os 10 a 20 dias pós-parto e a vaca continua em BEN até os 70 a 80 dias pós-parto e, em alguns casos, até aos 100 dias pós-parto (vacas primíparas). Entretanto, mesmo com BEN, uma proporção elevada das vacas inicia sua atividade ovariana normal nas primeiras 8 semanas pós-parto. As vacas que alcançam um nível baixo de BEN rapidamente e saem deste ponto mínimo de forma rápida, iniciam sua atividade ovariana mais rápido também (20 a 30 dias pós-parto) do que aquelas que têm um BEN mais profundo e duradouro, essas muitas vezes permanecem em anestro aos 70 dias pós-parto (figura 1). As mudanças na condição corporal estão correlacionadas positivamente com as concentrações séricas de insulina, IGF-I e leptina; assim, a maior qualificação da condição corporal é maior mediante a concentração sérica desses hormônios, que atuam principalmente como sinais que chegam ao hipotálamo e modificam a frequência de secreção do GnRH e, consequentemente, de LH. A leptina é um hormônio que é produzido pelos adipócitos e tem sido proposto que sua concentração é o sinal mais importante de alterações de condição corporal. A transição do anestro para a ciclicidade coincide com um aumento das concentrações séricas de insulina, IGF-I e leptina. Além das mensagens dadas pelos hormônios supracitados, outras substâncias presentes no sangue fornecem informações sobre o estado metabólico do animal. Por exemplo, os ácidos graxos não esterificados e o β-hidroxibutirato são indicadores da mobilização e utilização da gordura corporal; assim, o aumento destas substâncias proporciona uma mensagem inibidora de reprodução. O intervalo médio entre o parto e a primeira ovulação, em vacas leiteiras em sistemas de produção não intensiva, é em torno de 30 dias; enquanto em vacas em sistemas de produção intensiva, é de 40 dias. Deve-se sinalar que é comum aos 70 dias pós-parto, que 20% das vacas ainda não ovulem. Verifica-se frequentemente que as vacas leiteiras desenvolvem folículos que crescem mais que os folículos ovulatórios, os quais convertem em cistos foliculares. Esta condição está associada com insensibilidade do hipotálamo ao retorno positivo do estradiol, o que não desencadeia o pico pré-ovulatório de LH (figura 2). 3. O corpo lúteo dos primeiros ciclos pós-parto Durante um ciclo estral o corpo lúteo tem uma meia-vida fisiológica de 12 a 14 dias; no entanto, na primeira ovulação pós-parto cerca de 25% das vacas desenvolvem corpos lúteos de vida curta (<10 dias), 30% de vida longa (21 a 50 dias) e 45% da vida normal (11 a 20 dias). A regressão prematura do corpo lúteo é uma condição frequente nos ruminantes no primeiro ciclo estral, da transição do anestro à ciclicidade. Ou seja, na vaca é frequente que no primeiro ciclo de pós-parto e da puberdade sejam ciclos curtos; isto deve-se à liberação antecipada da PGF2α. Por outro lado, a causa dos corpos lúteos de vida longa (corpos lúteos persistentes) não é clara, mas está relacionada com alterações na secreção da PGF2α devido a infecções uterinas (tabela 1). 4. Anestro patológico 4.1 Alterações do aparelho genital que causam anestro Na vaca foi calculado que as alterações do aparelho genital que afetam a atividade ovariana representam apenas 10% do total das causas de anestro. Entre as alterações do aparelho genital se encontram os cistos foliculares, cistos luteinizados, aplasia segmentar, piometra e mumificação fetal. . 4.2 Cistos foliculares Os cistos foliculares são a patologia ovariana mais comum nos bovinos leiteiros, causando perdas econômicas devido ao atraso do período do parto ao primeiro serviço, pelo custo dos tratamentos e pelo risco que têm as vacas de serem descartadas. A incidência aumentou à medida que a produção de leite se intensificou. Entre 5 e 30% das vacas desenvolvem cistos foliculares nos primeiros 60 dias do pós-parto; no entanto, cerca de 60% destas se recuperam espontaneamente. Sinais clínicos de vacas com cistos foliculares descritos na literatura são: ninfomania, ciclos curtos, masculinização e relaxamento dos ligamentos pélvicos. Atualmente uma alta proporção de vacas com cistos foliculares apresentam anestro. Por muitos anos um cisto folicular foi definido como um folículo com um diâmetro de 2,5 mm, presente em um ou em ambos os ovários durante um período mínimo de 10 dias, na ausência de um corpo lúteo. Os conhecimentos atuais modificaram o conceito clássico; assim, nem todos os cistos foliculares têm um diâmetro de 2,5 mm; além disso, alguns podem persistir mais de 10 dias. Outros são estruturas dinâmicas, que sofrem regressão e podem ser substituídas por novos cistos. Por esta razão, a definição mais precisa e técnica de um cisto folicular é: folículo de um diâmetro, de pelo menos 20 mm, que está presente em um ou ambos os ovários na ausência de tecido lúteo que interfere com o ciclo estral normal (figura 3). A patogênese dos cistos foliculares não é conhecida. Propõe-se que as vacas que desenvolvem esta patologia têm uma anomalia nos mecanismos de retroalimentação entre o hipotálamo e a hipófise, em conjunto com uma disfunção a nível folicular. É pesquisado e sugerido que em vacas com cistos foliculares, o pico pré-ovulatório de LH não ocorre ou é de menor amplitude, ou não tem uma relação síncrona com o amadurecimento do folículo, o que provoca a falha ovulatória. A alteração na secreção de LH pode obedecer a falta de sensibilidade do hipotálamo ao feedback positivo de estrogênio. Observa-se também que algumas anormalidades a nível folicular, tais como alterações na síntese de hormônios esteroides e uma menor sensibilidade à LH, podem contribuir com a patogenia. Uma vez que o cisto folicular foi estabelecido, observou-se um aumento na frequência de secreção pulsátil de LH, o que contribui com a persistência desta patologia. Existem fatores associados com a incidência de cistos foliculares que, de acordo com a patogenia proposta, podem influenciar ao nível do eixo hipotálamo-hipófise-ovário. Os cistos foliculares ocorrem principalmente durante a transição do anestro pós-parto para a ciclicidade. Neste período as vacas estão em BEN, e tem sido observado que as vacas que sofrem de um BEN mais profundo tendem a apresentar uma incidência maior de cistos foliculares. Por outra parte, há uma correlação positiva entre a produção de leite e a incidência de cistos, o que indica que as vacas que produzem mais leite têm maior risco de sofrer com esta patologia. Também existem outros fatores relacionados com a incidência de cistos foliculares tais como estresse, genéticos, infecções uterinas, estresse calórico e presença de fitoestrogênios na dieta (tabela 2). O tratamento indicado consiste na administração do GnRH, que provoca a luteinização do cisto. É recomendável a combinação com uma dose luteolítica de PGF2α, 7 a 10 dias depois da injeção de GnRH. Também é indicado o tratamento com hCG em vez de GnRH. Em alguns estudos foi possível integrar vacas com cistos foliculares em programas de sincronização da ovulação e inseminação em tempo fixo, com resultados aceitáveis de fertilidade (figura 4). 4.3 Cistos luteinizados Esse tipo de cisto pode se desenvolver a partir de um cisto folicular, também ocorre quando o saco libera um óvulo e, em seguida, se fecha novamente e enche-se de líquido. O tecido luteinizado produz progestágenos do qual os sinais incluem anestro persistente. São bastante mais raros e originam-se pelo contínuo aumento do espaço cavitário central normalmente existente nos corpos lúteos (figura 5). Normalmente, a parede do cisto contém células lúteogranulosas grandes e células tecaluteais pequenas. Se diferenciam com dificuldade de corpos lúteos recém-formados durante o período em que ainda possuem um antro. Os cistos luteínicos únicos, pelo menos os menores, necessitam de importância clínica. Se os cistos são funcionais, a sintomatologia mais destacada é o anestro ou pseudogestação. É preciso lembrar que esses cistos agem como uma fonte de secreção contínua de progesterona, diminuindo assim a resistência do útero a infecções, podendo desencadear-se uma endometrite (figura 6). Se eles forem diagnosticados é recomendável a administração de PGF2α. 4.4 Aplasia segmentar As vacas com esta condição podem gestar quando a ovulação ocorre do lado do corno uterino presente. Se a ovulação acontece do lado onde falta o corno uterino causa anestro. 4.5 Piometra A piometra, por definição, é a acumulação de material purulento no interior da luz uterina que ocorrem geralmente durante ou imediatamente após o período de domínio da progesterona. É uma condição que ocorre mais frequentemente nos primeiros 30 dias pós-parto; no entanto, algumas vacas em anestro que são revisadas no dia 50 pós-parto chegam a ter piometra. O tratamento indicado consiste na administração de PGF2α. 5. Anestro relacionado com o tempo de serviço O anestro em bovinos também é classificado como anestro pré-serviço e pós-serviço. 5.1 Anestro pré-serviço Inclui vacas e novilhas que não tenham mostrado estro, isto é, manifestado cio, no período em que devem ser servidas. Em alguns estudos observou-se que o anestro antes do serviço pode afetar até 50% das vacas elegíveis para apresentar estro. 5.1 Anestro pós-serviço Neste grupo incluem-se as vacas que não retornam ao estro, ou seja, que não retornaram ao cio, 21 dias depois da inseminação e não estão prenhes. Essas vacas são conhecidas como vacas fantasma (The Phantom Cow). A causa principal do atraso do retorno ao estro é a baixa eficiência na detecção de estros; mas também foram descritas outras causas como: inseminação em um estro anovulatório, inseminação em tempo fixo em vacas em anestro, suspensão da ciclicidade após o serviço, fases lúteas longas e morte embrionária. O diagnóstico precoce da gestação é de grande ajuda para conhecer oportunamente o número de fêmeas que não estão prenhes. A ecografia no trigésimo dia pós-inseminação adianta, pelo menos 10 dias, a ressincronização de vacas vazias. 6. Falso anestro ou anestro funcional Este causa as maiores perdas econômicas de caráter reprodutivo em rebanhos leiteiros, e refere-se às vacas que estão ciclando, mas não são observadas em estro pelos trabalhadores. É um dos maiores problemas dentro de um sistema de criação extensivo e, às vezes, semiintensivo, uma vez que é de suma importância a observação técnica e precisa do comportamento da vaca para observar o possível retorno ao cio ou não. 7. Manejo da vaca anéstrica A probabilidade de que a vaca seja inseminada oportunamente depende da eficiência da detecção de estros. É comum que a metade das vacas apresentem estro e não sejam observadas, por essa razão, muitas não são inseminadas uma vez que termina o período de espera voluntária. Por outro lado, há vacas que, por causas patológicas ou por seu estado metabólico, ainda estão em anestro depois que termina o período voluntário de espera. Para identificar as causas da ausência de estros, todas as vacas que não tenham sido inseminadas no dia 60 pós-parto, devem ser revisadas por via retal para aplicar o tratamento ou manejo pertinentes. Durante esta revisão é dada especial atenção às características do útero e nas estruturas ovarianas, já que daqui depende o manejo subsequente. A palpação começa no útero; neste é importante determinar se não há gestação. Posteriormente, avalia-se sua consistência que pode ser normal, edematosa ou turgente. Depois de avaliar o útero procede-se a palpação dos ovários, começando com o direito e, em seguida, esquerdo. Abaixo estão descritas as diferentes descobertas que podem ser encontradas na literatura e seu tratamento ou manejo. O registo dos resultados à palpação é feito através de chaves reprodutivas. 7.1 UN CLD2-3 FE10 Útero normal (UN) com um corpo lúteo 2 ou 3 no ovário direito (CLD2-3) e um folículo no ovário esquerdo de 10 mm de diâmetro (FE10). A consistência normal do útero (é normal quando não há edema ou turgescência) é encontrado em vacas não prenhes durante o diestro, ou em vacas que estão em anestro. O CL 2 ou 3 é uma estrutura bem desenvolvida que deforma o ovário e em alguns casos representa mais de 50% da massa ovariana. Classificá-los como CL 2 ou 3 é uma apreciação subjetiva do tamanho do corpo lúteo e não tem significado prático, pois em qualquer dos casos o manejo é o mesmo. O CL indica que a vaca está em qualquer dia do diestro e, obviamente, indica que está ciclando. Durante o diestro pode-se encontrar folículos de tamanho diferente em qualquer um dos dois ovários, já que este depende das ondas de desenvolvimento folicular. É importante frisar que as estruturas mencionadas podem estar em ovários diferentes ou no mesmo ovário. O achado mais importante nesta fase é a presença do corpo lúteo o que permite o tratamento com a PGF2α, o que resulta na apresentação do estro nas próximas 48 a 120 horas. A presença de um corpo lúteo é o estado fisiológico que é mais frequentemente encontrado neste grupo de vacas, primeiro porque paradoxalmente a maior proporção de vacas em “anestro” está ciclando (anestro funcional) e, em segundo lugar, porque o diestro ocupa 65% dos dias do ciclo estral. 7.2 UE CLD1 FE10-15 Útero edematoso (UE) com um corpo lúteo 1 no ovário direito (CLD1) e um folículo 10 ou 15 mm de diâmetro no ovário esquerdo. O útero edematoso pode ser encontrado no proestro e metaestro. A presença do CL1 e um folículo grande indica que se trata de uma vaca que muito provavelmente é encontrado em proestro. A diferença entre um CL1 e um CL 2 ou 3 é basicamente o seu tamanho; um CL1 é uma estrutura pequena com consistência dura. As vacas que têm estas características devem ser marcadas para que os trabalhadores prestem mais atenção, já que apresentarão o estro nas próximas 48 a 72 horas. Se a vaca não apresentar estro nesse período, deverá ser palpada na semana seguinte. 7.3 UT DE FE10-15 Útero túrgido ou tonificado, ovário direito estático e ovário esquerdo com um folículo de 10 ou 15 mm de diâmetro. Estes achados, além da presença de muco estral, correspondem a uma vaca em estro. Frequentemente, na palpação das vacas do grupo de anestro, há vacas em estro; estas vacas deverão ser programadas para inseminação. 7.4 UE DE EE Útero estático e ovários estáticos. Estas observações correspondem a uma vaca em metaestro; esta decisão tem uma alta margem de erro uma vez que também pode corresponder a um animal no proestro ou em anestro verdadeiro. Um achado que permite ser mais bem sucedido no diagnóstico é a presença de sangue no muco cervical; neste caso, a presença de sangue indica, com certeza, que a vaca está em metaestro; no entanto, nem todas as vacas apresentam este sangramento. As vacas com estes achados devem ser palpadas sete dias depois para confirmar ou corrigir um primeiro diagnóstico. Se a primeira palpação foi correta, na segunda se encontrará um CL2-3. 7.5 UE CHD FE10 Útero edematoso, com corpo hemorrágico no ovário direito (CHD) e com folículo de 10 mm de diâmetro no ovário esquerdo. Estas observações são de uma vaca em metaestro. O corpo hemorrágico é considerado como a fase de transição entre o folículo e o corpo lúteo; o CH é palpado como uma estrutura com uma saliência em forma de papila e é muito suave ao toque. Será necessário esperar 4 ou 5 dias para que se converta num corpo lúteo maduro e assim destruí-lo com PGF2α. Na rotina, estas vacas são palpadas na seguinte revisão (sete dias depois). 7.6 UN DE EE Útero normal e ovários estáticos. Este caracteriza as vacas que estão no anestro verdadeiro. As vacas caem no anestro principalmente por se encontrar em balanço negativo de energia; este problema é mais grave em vacas de primeiro parto. O único tratamento eficaz consiste em melhorar o seu estado metabólico. Tratamentos hormonais não funcionam caso não seja resolvido primeiro o seu estado nutricional. 7.7 UN CFD EE Útero normal, cisto folicular no ovário direito e ovário esquerdo estático. Embora as vacas com cistos foliculares sejam caracterizadas por apresentar estros recorrentes, também chegam a apresentar anestro. O tratamento consiste na administração do GnRH ou hCG. 7.8 UN CLD EE Útero normal, cisto luteinizado no ovário direito e ovário esquerdo estático. Este cisto também é causado por uma deficiência na secreção de LH, só que neste caso a deficiência foi parcial, o que provoca um certo grau de luteinização. O cisto luteinizado é uma estrutura com mais de 20 mm de diâmetro de paredes grossas. O tratamento indicado é a administração de PGF2α. Na prática, é difícil diferenciar um cisto folicular de um luteinizado, pelo que o tratamento recomendado é, primeiro, a administração de GnRH ou hCG e sete dias depois é injetado PGF2α (figura 7). 7.9 Piometra CLD2-3 FE A piometra é uma condição que ocorre principalmente nos primeiros 30 dias pós-parto; no entanto, na revisão das vacas anéstricas são encontradas vacas com esta patologia. O tratamento indicado consiste na administração de PGF2α. 8. Tratamentos hormonais para indução da atividade ovariana O estabelecimento precoce da ciclicidade pós-parto favorece a involução uterina e está positivamente correlacionado com a fertilidade; ou seja, quanto mais ciclos estrais a vaca tenha antes da primeira inseminação, a percentagem de concepção é maior. Um tratamento utilizado para induzir a primeira ovulação pós-parto em vacas anéstricas, consiste na administração de GnRH quando à palpação retal se encontra um folículo grande (> 10 mm). Com isso se pretende fazer o folículo ovular e depois injeta-se PGF2α para provocar a regressão lútea. Outras combinações consistem na administração de GnRH e na inserção de dispositivos de liberação de progesterona e, ao retirar o progestágeno, injeta-se PGF2α (figura 8). No entanto, os tratamentos mencionados são de uso comum, não funcionam em todos os casos. Um requisito para se obter êxito é que as vacas devem ter boa condição corporal ou que estejam ganhando condição corporal. Embora os tratamentos hormonais possam ajudar em alguns casos, não deve-se esquecer que as causas do anestro não são corrigidos apenas com a administração de hormônios, já que a vaca não cicla porque toda a informação do estado metabólico que recebe o cérebro, indica que não o deve fazer. 9. Gonadotropina coriônica equina (eCG) Na égua, por volta do dia 30 de gestação, células do córion migram para o endométrio e formam umas estruturas conhecidas como copas endometriais. Entre o dia 40 e o dia 130 de gestação nestas estruturas é produzido o eCG (anteriormente conhecida como gonadotropina sérica da égua prenha ou PMSG, pela sua abreviatura em inglês). Na égua esse hormônio tem atividade de LH pelo qual estimula a função do corpo lúteo e promove a formação de corpos lúteos acessórios. Nos ruminantes, o eCG se une aos receptores de LH e FSH do folículo, estimulando o desenvolvimento folicular, e no corpo lúteo estimula a secreção de progesterona. O eCG tem sido utilizado com a finalidade de superovulação e em vacas de corte sob anestro é incluído em programas com progestágenos para induzir ciclicidade. Nestes programas é injetado eCG no momento da remoção do progestágeno, o que favorece o desenvolvimento folicular e apresentação do estro. Em rebanhos leiteiros na Nova Zelândia tem sido incrementado a taxa de prenhez sem aumentar a proporção de partos gêmeos, através da injeção de eCG na remoção do dispositivo de libertação de progesterona em programas combinados com a injeção de benzoato de estradiol. RESUMO ➢ Transcorrem 15 dias pós-parto para que a hipófase responda ao hormônio GnRH. ➢ Entre os dias 12 e 15 pós-parto começa-se a secreção de FSH e iniciam as ondas foliculares. ➢ As vacas chegam ao ponto mais baixo de BEN entre os dias 15 e 20 pós-parto. ➢ As vacas iniciam a atividade ovariana normal, em média, após os 40 dias pós-parto. ➢ Aos 70 dias pós-parto é normal que 20% das vacas ainda estejam em anestro. ➢ Na primeira ovulação pós-parto 25% das vacas desenvolvem corpos lúteos de vida curta e 30% mostram corpos lúteos de vida longa (persistentes). ➢ As anormalidades do aparelho reprodutor são responsáveis por 10% das causas de anestro. ➢ 5 a 30% das vacas desenvolvem cistos foliculares nos primeiros 60 dias pós-parto e 60% destas se recuperam de forma espontânea. ➢ No dia 30 pós-inseminação é possível identificar vacas vazias mediante a ecografia e reservá-las para a ressincronização. ➢ A proteína B específica da gestação aparece no soro a partir do 15º dia de gestação. ➢ Para o diagnóstico precoce de gestação é recomendado medir a PSPB no dia 30 pósinseminação e em vacas com mais de 90 dias pós-parto. ➢ Entre os dias 50 e 60 pós-parto todas as vacas não servidas devem ser examinadas via retal para programar seu serviço. In this paper I defend an eudaimonistic reading of Spinoza’s ethical philosophy. Eudaimonism refers to the mainstream ethical tradition of the ancient Greeks, which considers happiness a naturalistic, stable, and exclusively intrinsic good. Within this tradition, we can also draw a distinction between weak eudaimonists and strong eudaimonists. Weak eudaimonists do not ground their ethical conceptions of happiness in complete theories of metaphysics, epistemology, or psychology. Strong eudaimonists, conversely, build their conceptions of happiness around an overall philosophical system that extends (...) far beyond ethics, while nevertheless being directed at the promotion of a happy life. I will show that Spinozistic happiness is not only naturalistic, stable, and exclusively intrinsically good, but that Spinoza is also a strong eudaimonist because his ethical account of happiness is incomprehensible without appeal to metaphysical, epistemological, and psychological doctrines. As well, I will explain how the apparent subjective and relativistic features of Spinoza’s ethics do not undermine the eudaimonistic reading, because both Spinoza and the ancient eudaimonists grant that the beliefs/feelings of the subject play a necessary (but insufficient) role in happiness as the highest good. Objective: In this study, we aimed to identify patients at risk of chemotherapy-induced cardiotoxicity with a simple method like platelet-to-lymphocyte ratio (PLR) before starting therapy. Method: A total of 65 breast cancer patients who completed anthracycline or adjuvant trastuzumab treatment were evaluated retrospectively. Serial PLR calculations, echocardiographic examinations, and cardiac markers before treatment and after follow-up period were analyzed. Cardiotoxicity was determined according to Cardiac Review and Evaluation Committee Criteria. Results: Patients were divided into two groups according to their (...) baseline PLR levels as Group C—PLR < 119 and Group D—PLR 120. The median follow-up of the study was 22.23 (12-42) months. Concomitant disease and baseline characteristics were similar in both groups. Symptomatic cardiotoxicity was not observed in both groups. Cardiotoxicity was occurred in one patient (2.3%) in Group C and in four patients (9.5%) in Group D (P ¼ .005). Average mean left ventricular ejection fraction loss from baseline was 10.7 6 7.0% in Group D vs 2.3 6 6.4% in Group C (P ¼ .008). Interpretation of cardiac markers that were present in nearly half of the patients revealed that serum hs-c-reactive protein and pro-brain natriu¨ retic peptide levels were significantly higher in patients who developed cardiotoxicity compared to who did not develop cardiotoxicity. PLR 120 had 99% sensitivity and 85% specificity in predicting cardiotoxicity. Conclusion: This study’s results showed that high PLR levels were associated with chemotherapy-induced cardiotoxicity. To our best knowledge, this is the first study, examining the impact of whole blood test on chemotherapy-induced cardiotoxicity before starting the therapy and allowing doctors plot a route for these risky patients. Introduction: Universal health coverage (UHC) was introduced in Iran in 2014. The aim of this study was to evaluate the usage rate of health services by human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) patients after UHC implementation. Material and methods: In 2018, in a cross-sectional study, we evaluated the outpatients’ needs (within its previous month) and inpatients’ needs (within its previous 6 months) of HIV/AIDS patients in Isfahan province (the center of Iran). Concurrently, we estimated the essential health care services (...) that HIV/AIDS patients have to receive regularly, including vaccination for hepatitis B, measurement of CD4, tuberculosis (TB) assessments and TB treatment, anti-retroviral therapy, examination of viral load, treatment approach, and drug side effects counseling. Two checklists were used for assessing the utilization of health services and essential health cares for HIV/AIDS patients, validated by the Ministry of Health and Medical Education of Iran. Data were analyzed by χ2 test, Pearson’s correlation coefficient, and Spearman’s correlation test. Results: Two hundred and thirteen HIV/AIDS patients completed the questionnaires. The mean age of participants was 41.14 ± 9.23. The outpatient service utilization rate was 31.94% in the previous month and the rate of hospitalization was 126 per 1,000 HIV/AIDS patients in the previous 6 months. The majority of HIV/AIDS patients received essential health services more often than the national standard goals estimation. Conclusions: After UHC implementation, the utilization rate of outpatients and inpatients services in HIV/AIDS patients was more than similar indices in the general population. The theoretical underpinnings and practical worth of content-based, intentional, or "folk" psychology have been challenged by three distinct groups of philosophical critics in the past 15 years or so. The first group, comprised by Hilary Putnam, Tyler Burge, and other advocates of "wide" or "externalist" theories of meaning, claims that traditional psychologists have been mistaken in assuming that our beliefs, desires, and other content-laden states supervene on or inhere in our individual minds or brains. The other two groups are both (...) "eliminative materialists," who charge that the intentional approach is inadequate and that it can or will be replaced by a completely non-interpretive discipline: either neuropsychology, in the view of Patricia and Paul Churchland, or a strictly syntactic computational psychology, according to Stephen Stich. ;This dissertation defends "notional world" or narrow intentional psychology against these charges, primarily on the strength of its practical merits, in contrast to the limitations and adverse effects of the proposed alternatives. Psychology is at least partly an applied science with a mandate to help understand and treat concrete psychological problems such as Post Traumatic Stress Disorder and depression, I argue, so any theorist who proposes to reconfigure or phase out existing approaches must be prepared to take over these duties with at least equal facility. However, whereas various "narrow" schools of psychotherapy such as Cognitive Therapy are fairly successful in this regard and show every indication of continuing to be needed for the foreseeable future, the Syntactic Theory seems to show very poor promise of being able to help relieve the distress of people with psychological disturbances, while a purely neurobiological approach is inappropriate in many cases, and tends to cause a variety of untoward and dangerous side-effects. As for the "wide" theorists with their emphasis upon the social and environmental contributions to meaning: they must acknowledge that a good deal of content is in the head; and, more importantly, by focusing on the role of the "experts" in a society's language-game, they miss the whole point of a psychological attribution, which is to understand an individual's reasons--however idiosyncratic--for acting as he or she does. After tracing the historical origin of philosophy cafés, as part of the worldwide philosophical practice movement, this article explains how the Hong Kong Philosophy Café was founded and describes a typical meeting. During its first year of existence, an Executive Committee was formed, which oversaw the setting up of eight different branches over the next ten years. Following sections that describe the work of the Executive Committee and the distinctive features of eight different branches, the article concludes with a summary (...) of the current situation and reflections on how recent developments in social media are likely to affect the way such public meetings are organized, as well as their continued impact on the social and political engagement of Hong Kong citizens. Amodal completion is usually characterized as the representation of those parts of the perceived object that we get no sensory stimulation from. In the case of the visual sense modality, for example, amodal completion is the representation of occluded parts of objects we see. I argue that relationalism about perception, the view that perceptual experience is constituted by the relation to the perceived object, cannot give a coherent account of amodal completion. The relationalist has two options: construe (...) the perceptual relation as the relation to the entire perceived object or as the relation to the unoccluded parts of the perceived object. We argue for the following three claims: First, at least some amodal completion-involved experiences can ground knowledge about the occluded portions of perceived objects. Second, at least some instances of amodal completion-grounded knowledge are not sensitive, that is, it is not the case that in the nearest worlds in which the relevant claim is false, that claim is not believed true. Third, at least some instances of (...) amodal completion-grounded knowledge are not safe, that is, it is not the case that in all or nearly all near worlds where the relevant claim is believed true, that claim is in fact true. Ronald Dworkin argued that Advance Directives informed by a principle of autonomy ought to guide decisions in relation to the treatment of those in care for dementia. The principle of autonomy in play presupposes a form of competence that is tied to the individual person making the Directive. This paper challenges this individualist assumption. It does so by pointing out that the competence of a patient is inherently relational, and the key illustrative case to make this point is the case (...) of music therapy. In music therapy, a relatively recent treatment modality in aged care, patients previously thought to be permanently unresponsive are shown on the contrary to be capable of significant levels of social agency. The conclusion to draw is that Advance Directives that fail to acknowledge the real possibility of such relational competence are misapplied. (shrink)

An important strand of current experimental philosophy promotes a new kind of methodological naturalism. This chapter argues that this new ‘metaphilosophical naturalism’ is fundamentally consistent with key tenets of Wittgenstein’s metaphilosophy, and can provide empirical foundations for therapeutic conceptions of philosophy. Metaphilosophical naturalism invites us to contribute to the resolution of philosophical problems about X by turning to scientific findings about the way we think about X – in general or when doing philosophy. This new naturalism encourages us to use (...) resources from psychology that can empirically vindicate precisely some of the most controversial aspects of Wittgenstein’s conception of philosophy: They can establish the need, and provide key tools, for something worth calling ‘therapy’, in philosophy. As ‘pudding proof’ this chapter shows how methods and findings from psycholinguistics motivate and facilitate a therapeutic approach to a characteristically philosophical problem: ‘the problem of perception’. Sports physicians are continuously confronted with new biotechnological innovations. This applies not only to doping in sports, but to all kinds of so-called enhancement methods. One fundamental problem regarding the sports physician's self-image consists in a blurred distinction between therapeutic treatment and non-therapeutic performance enhancement. After a brief inventory of the sports physician's work environment I reject as insufficient the attempts to resolve the conflict of the sports physician by making it a classificatory problem. Followed by a critical assessment of (...) some ideas from the US President's Council on Bioethics, the formulation of ethical codes and attempts regarding a moral topography, it is argued that the sports physician's conflict cannot be resolved by the distinction between therapy and enhancement. Instead, we also have to consider the possibility that the therapy-based paradigm of medicine cannot do justice to the challenges of the continuously increasing technical manipulability of the human body and even our cognitive functions as well. At the same time we should not adhere to transhumanist ideas, because non-therapeutic interventions require clear criteria. Based on assistive technologies an alternative framework can be sketched that allows for the integration of therapeutic and non-therapeutic purposes. After a thorough definition of standards and criteria, the role of the sports physician might be defined as that of an assistant for enhancement. Yet the process of defining such an alternative framework is a societal and political task that cannot be accomplished by the sports physicians themselves. Until these questions are answered sports physicians continue to find themselves in a structural dilemma that they partially can come to terms with through personal integrity. (shrink)

Investigation of a puzzle concerning complete yet authorless artworks.

We investigate a recently devised polyhedral semantics for intermediate logics, in which formulas are interpreted in n-dimensional polyhedra. An intermediate logic is polyhedrally complete if it is complete with respect to some class of polyhedra. The first main result of this paper is a necessary and sufficient condition for the polyhedral completeness of a logic. This condition, which we call the Nerve Criterion, is expressed in terms of Alexandrov’s notion of the nerve of a poset. It affords a purely combinatorial (...) characterisation of polyhedrally complete logics. Using the Nerve Criterion we show, easily, that there are continuum many intermediate logics that are not polyhedrally complete but which have the finite model property. We also provide, at considerable combinatorial labour, a countably infinite class of logics axiomatised by the Jankov–Fine formulas of ‘starlike trees’ all of which are polyhedrally complete. The polyhedral completeness theorem for these ‘starlike logics’ is the second main result of this paper. (shrink)

The paper clarifies therapeutic ideas about philosophical method which Wittgenstein puts forward in his "Big Typescript". It does so by analysing how Wittgenstein treats the question 'What is meaning?', in that part of the same work from which the opening sections of his "Philosophical Investigations" derive. On this basis, the paper explains why Wittgenstein set himself a therapeutic goal, why this is reasonable, and how he sought to attain that goal without 'pronouncing new truths about the subject of the investigation', (...) viz. meaning. Well-known results due to David Makinson show that there are exactly two Post complete normal modal logics, that in both of them, the modal operator is truth-functional, and that every consistent normal modal logic can be extended to at least one of them. Lloyd Humberstone has recently shown that a natural analog of this result in congruential modal logics fails, by showing that not every congruential modal logic can be extended to one in which the modal operator is truth-functional. As (...) Humberstone notes, the issue of Post completeness in congruential modal logics is not well understood. The present article shows that in contrast to normal modal logics, the extent of the property of Post completeness among congruential modal logics depends on the background set of logics. Some basic results on the corresponding properties of Post completeness are established, in particular that although a congruential modal logic is Post complete among all modal logics if and only if its modality is truth-functional, there are continuum many modal logics Post complete among congruential modal logics. Computational complexity is a discipline of computer science and mathematics which classifies computational problems depending on their inherent difficulty, i.e. categorizes algorithms according to their performance, and relates these classes to each other. P problems are a class of computational problems that can be solved in polynomial time using a deterministic Turing machine while solutions to NP problems can be verified in polynomial time, but we still do not know whether they can be solved in polynomial time as well. A (...) solution for the so-called NP-complete problems will also be a solution for any other such problems. Its artificial-intelligence analogue is the class of AI-complete problems, for which a complete mathematical formalization still does not exist. In this chapter we will focus on analysing computational classes to better understand possible formalizations of AI-complete problems, and to see whether a universal algorithm, such as a Turing test, could exist for all AI-complete problems. In order to better observe how modern computer science tries to deal with computational complexity issues, we present several different deep-learning strategies involving optimization methods to see that the inability to exactly solve a problem from a higher order computational class does not mean there is not a satisfactory solution using state-of-the-art machine-learning techniques. Such methods are compared to philosophical issues and psychological research regarding human abilities of solving analogous NP-complete problems, to fortify the claim that we do not need to have an exact and correct way of solving AI-complete problems to nevertheless possibly achieve the notion of strong AI. In previous articles, it has been shown that the deductive system developed by Aristotle in his "second logic" is a natural deduction system and not an axiomatic system as previously had been thought. It was also stated that Aristotle's logic is self-sufficient in two senses: First, that it presupposed no other logical concepts, not even those of propositional logic; second, that it is (strongly) complete in the sense that every valid argument expressible in the language of the system is deducible (...) by means of a formal deduction in the system. Review of the system makes the first point obvious. The purpose of the present article is to prove the second. Strong completeness is demonstrated for the Aristotelian system. (shrink)

In this opinion piece we propose the investigation of psychedelic-assisted psychotherapy for the treatment of body dysmorphic disorder (BDD). BDD is a psychiatric disorder characterised by appearance-based preoccupations and accompanying compulsions. While safe and effective treatments for BDD exist, non-response and relapse rates remain high. Therefore, there is a need to investigate promising new treatment options for this highly debilitating condition. Preliminary evidence suggests safety, feasibility, and potential efficacy of psychedelic treatments in disorders that share similar psychopathological mechanisms with BDD. (...) Drawing on this evidence, as well as on relevant qualitative reports and theoretical proposals, we argue that it would be worthwhile to conduct a phase 2a study aimed at assessing the safety and feasibility of psychedelic-assisted psychotherapy in BDD. We give a new and elementary construction of primitive positive decomposition of higher arity relations into binary relations on finite domains. Such decompositions come up in applications to constraint satisfaction problems, clone theory and relational databases. The construction exploits functional completeness of 2-input functions in many-valued logic by interpreting relations as graphs of partially defined multivalued ‘functions’. The ‘functions’ are then composed from ordinary functions in the usual sense. The construction is computationally effective and relies on well-developed methods of functional (...) decomposition, but reduces relations only to ternary relations. An additional construction then decomposes ternary into binary relations, also effectively, by converting certain disjunctions into existential quantifications. The result gives a uniform proof of Peirce’s reduction thesis on finite domains, and shows that the graph of any Sheffer function composes all relations there. (shrink)

Desire is an important philosophical topic that deeply impacts everyday life. Philosophical practice is an emerging trend that uses philosophical theories and methods as a guide to living a eu‐ daimonic life. In this paper, we define desire philosophically and compare different theories of desire in specific Eastern and Western traditions. Based on the Lacanian conceptual–terminological triad of “Need‐Demand‐Desire”, the research of desire is further divided into three dimensions, namely, the subject of desire, the object of desire, and the desire (...) itself. The concept of desire is then an‐ alyzed from this triad and these three dimensions through different philosophical theories. This paper selects Buddhism as the representative of Eastern tradition, and Stoicism as the representative of the West, paying special attention to Stoicism’s “spiritual exercises” following Pierre Hadot. By exploring and comparing the Buddhist paths to liberation from suffering (i.e., the Four Noble Truths and the Noble Eightfold Path) and the two theoretical pillars in Stoicism (i.e., the notions of “living according to nature” and “the dichotomy of control”), practical guidance is then provided for un‐ derstanding and regulating desire in times of crisis. This understanding and regulation of desire constitutes a philosophical therapy for today’s troubles, particularly those caused by excessive or irrational desires. (shrink)

Defense of a psychological account of what it is for an artwork to be complete.

In his article Banicki proposes a universal model for all forms of philosophical therapy. He is guided by works of Martha Nussbaum, who in turn makes recourse to Aristotle. As compared to Nussbaum’s approach, Banicki’s model is more medical and less based on ethical argument. He mentions Foucault’s vision to apply the same theoretical analysis for the ailments of the body and the soul and to use the same kind of approach in treating and curing them. In his interpretation (...) of philosophical therapy, there are, however, some controversial issues, to which we would like to call attention: -/- Is restoring health by a philosophical method of treatment – health understood as a person’s ability to reach his/her vital goals – a convincing explication of philosophical therapy in general? In order to answer this question, it may be useful to look at Plato. It is not only Platonism (and especially Neo-platonism since Plotinus) that questions the idea that therapy is necessarily connected with „vital goals“. Buddhist and Gnostic philosophies are questioning „the vital“ in general. The immense effort in the history of philosophy to liberate the mind from the body casts doubt on the project to explain philosophical therapy solely in analogy to medical therapy. -/- According to Banicki a therapeutic philosophy has to identify the diseases it attempts at curing. There are, however, reasons for associating the term therapy with suffering/risk rather than disease. If a therapy aims at reducing the fear of death, as for example many classical philosophical therapies do, then there is no disease to be cured. A model based on chances and risks also has the advantage that the Buddhist and Stoic reinterpretation of desires/emotions as “diseases” can be dropped in contemporary philosophical therapy. -/- Banicki mentions the comparison of therapies as one of the primary goals of modelling. Methods should be verified by a statistical correlation between method and therapeutic success and/or by a theory which justifies the method. Some forms of therapies, however, avoid theory-specific terms and concepts in favor of an unprejudiced interpretation of the patient’s statements. Others share the (Nietzschean) aspiration to explore and change measures of value. If the therapeutic process is seen as a unique phenomenon, then there can be no theory and no statistics with co-occurrences. -/- According to Banicki philosophical therapy has to describe techniques which qualify as genuinely philosophical. Techniques like maieutics and hermeneutics, however, are antique philosophical “inventions”, which were later adopted by psychotherapy. If meditation is accepted as a therapeutic tool – as it was in ancient times – then there is not only a methodical but also an emotional relation to religion. If finally, the Socratic search for a good life is seen as one of the characteristics of philosophical therapy, then delimitation even turns into a criterion for non-philosophical therapies. The search for a good life requires an interdisciplinary approach. -/- For the concretization of the formal structure, we suggest a typology of therapies, which is based on chances and risks. Such a typology has the advantage, that it can model conflicting (and even opposing) forms of therapies. The variety of therapeutic goals and methods is a consequence of the historical development towards conceptual freedom. (shrink)

We show that early vision can use monocular cues to rapidly complete partially-occluded objects. Visual search for easily detected fragments becomes difficult when the completed shape is similar to others in the display; conversely, search for fragments that are difficult to detect becomes easy when the completed shape is distinctive. Results indicate that completion occurs via the occlusion-triggered removal of occlusion edges and linking of associated regions. We fail to find evidence for a visible filling-in of contours or surfaces, (...) but do find evidence for a "functional" filling-in that prevents the constituent fragments from being rapidly accessed. As such, it is only the completed structures—and not the fragments themselves—that serve as the basis for rapid recognition. (shrink)

Amodal completion is the representation of those parts of the perceived object that we get no sensory stimulation from. While amodal completion is rife and plays an essential role in all sense modalities, philosophical discussions of this phenomenon have almost entirely been limited to vision. The aim of this paper is to examine in what sense we can talk about amodal completion in olfaction. We distinguish three different senses of amodal completion – spatial, temporal and feature-based (...) completion – and argue that all three are present and play a significant role in olfaction. In the Eudemian Ethics II 1, 1219a34–b8, Aristotle defines happiness as ‘the activity of a complete life in accordance with complete virtue’. Most scholars interpret a complete life as a whole lifetime, which means that happiness involves virtuous activity over an entire life. This article argues against this common reading by using Aristotle’s notion of ‘activity’ (energeia) as a touchstone. It argues that happiness, according to the Eudemian Ethics, must be a complete activity that reaches its end at any and (...) every moment. The upshot of this reading is that life reaches completeness within a lifetime and that death cannot be the requirement for making life complete. (shrink)

The U.S. FDA has issued emergency use authorizations for monoclonal antibodies for non-hospitalized patients with mild or moderate COVID-19 disease and for individuals exposed to COVID-19 as post-exposure prophylaxis. One EUA for an oral antiviral drug, molnupiravir, has also been recommended by FDA’s Antimicrobial Drugs Advisory Committee, and others appear likely in the near future. Due to increased demand because of the Delta variant, the federal government resumed control over the supply and asked states to ration doses. As future variants (...) with increased infectivity and/or severity emerge, further rationing may be required. We identify relevant ethical principles and priority groups for access to therapies based on an integrated approach to population health and medical factors. Using priority categories to allocate scarce therapies effectively operationalizes important ethical values. This strategy is preferable to the current approach of categorical rules based on vaccination, immunocompromise status, or older age, or the ad hoc consideration of clinical risk factors. (shrink)

The universe of discourse is the set of evangelists. We presuppose standard first-order logic. -/- As many logic texts teach, the first of these two premise-conclusion arguments—sometimes called a complete enumerative induction— is invalid in the sense that its conclusion does not follow from its premises. In this paper, we apply the perspective of intra-organismal ecology by investigating a family of ecological models suitable to describe a gene therapy to a particular metabolic disorder, the adenosine deaminase deficiency (ADA-SCID). The gene therapy is modeled as the prospective ecological invasion of an organ (here, bone marrow) by genetically modified stem cells, which then operate niche construction in the cellular environment by releasing an enzyme they synthesize. We show that depending on the chosen order (a choice (...) that cannot be made on \textit{a priori} assumptions), different kinds of dynamics are expected, possibly leading to different therapeutic strategies. This drives us to discuss several features of the extension of ecology to intra-organismal ecology. (shrink)

Philosophy as conceptual therapy, on the other hand, seeks an improvement of intellectual skills, and fosters a therapy for concepts and, by inference, a therapy for thinkers. The major concern of the paper is to argue that the traditional information-oriented approach to philosophy does not help its students effectively to develop intellectual skills, and that a conception of philosophy as conceptual therapy can provide a valuable contribution to the teaching and to the subject-matter of philosophy. We investigate a lattice of conditional logics described by a Kripke type semantics, which was suggested by Chellas and Segerberg – Chellas–Segerberg (CS) semantics – plus 30 further principles. We (i) present a non-trivial frame-based completeness result, (ii) a translation procedure which gives one corresponding trivial frame conditions for arbitrary formula schemata, and (iii) non-trivial frame conditions in CS semantics which correspond to the 30 principles.
