As part of this effort, the breast-milk substitutes industry is proposing to use renewable (...) gas as a substitute for fossil natural gas. However, decarbonising the industrial processing of breast-milk substitutes can increase social license for these products, potentially undermining breastfeeding. World Health Organisation nutrition targets aim to increase exclusive breastfeeding to at least 50% globally by 2025 to improve maternal, infant, and young child health and nutrition. This target will have implications for the energy transition. A weakness of existing energy models is that demands for end-use products such as breast-milk substitutes are typically not considered explicitly. This paper develops an analytical framework for explicitly representing infant feeding methods in energy systems models. We compare the emissions saved in Ireland from decarbonising the industrial processing of breast-milk substitutes with renewable gas with the emissions saved by an increase in exclusive breastfeeding to 50% in both Ireland and a key export market, China. We demonstrate that the emissions saved from achieving the minimum global breastfeeding target are greater than when renewable gas is used to displace natural gas in the production of breast-milk substitutes in Ireland. We discuss the decarbonisation of breast-milk substitutes in relation to the principle of justice as non-maleficence, a principle based on the commitment to avoid harm, a novel application of a principle of justice. We conclude that breastfeeding support can be considered a demand-side measure for mitigating climate change by reducing the demand for energy services to produce breast-milk substitutes. A key recommendation is to position breastfeeding support as both a public health and a climate justice issue that is relevant for a just transition. The framework developed for this paper could be applied to support the inclusion of a wider range of mitigation options with social justice outcomes in energy system models. [Open access]. (shrink)

Abstract: Breast cancer is reported to be the most common cancer type among women worldwide and it is the second highest women fatality rate amongst all cancer types. Notwithstanding all the progresses made in prevention and early intervention, early prognosis and survival prediction rates are still not sufficient. In this paper, we propose an ANN model which outperforms all the previous supervised learning methods by reaching 99.57 in terms of accuracy in Wisconsin Breast Cancer dataset. Experimental results on (...) Haberman’s Breast Cancer Survival dataset show the superiority of proposed method by reaching 88.24 % in terms of accuracy. The results are the best reported ones obtained from Artificial Neural Network using JNN environment without any preprocessing of the dataset. (shrink)

Globally, breast cancer is the most common cancer in women worldwide, it represents about 25.0% of all cancer cases. In developing countries, breast cancer is mainly discovered at a late stage, which has a negative impact on the prognosis. To determine the extent of, and the factors contributing to, delay in breast cancer presentation, and to evaluate the mammography screening practice in breast cancer Libyan women. A descriptive cross-sectional study was carried out on five hundred and (...) one breast cancer patients who attended the Oncology Outpatient Clinic of the Medical Department at Tripoli University Hospital and Tripoli Central Hospital during a period between February and August 2019, after the application of inclusion and exclusion criteria enrolled in the study. The mean age of the patients was 47.35±01.0 years, 55.1% of them were diagnosed in late stage (III and IV), and 30.9% of the patients were presented late (≥3 months). The time interval between the initial breast symptom and first presentation to a clinic ranged between 2nd day and up to three years, with a median time of 1.5 months, and the main factor of the delayed presentation to a clinic was a lack of knowledge about symptoms (33.5%). The study revealed a significant association between delayed presentation and older age (P<0.001), low educational level (P<0.001), low family income (P<0.001), unemployed women (P< 0.002), postmenopausal women (P<0.001), patients with negative family history (P<0.05). This study concluded that delayed presentation is found in less than a third of the study population, and lack of knowledge about breast cancer signs and symptoms is a common factor. Age, educational status, family income, unemployed women, menopausal status, and family history of breast cancer were all significant factors for delayed presentation. Improving women's awareness about breast cancer is needed and applying the policy of regular screening mammograms is a useful tool for early diagnosis and early treatment in Libya. (shrink)

Abstract: The Knowledge-Based System for Diagnosing Breast Cancer aims to support medical students in enhancing their education regarding diagnosis and counseling. The system facilitates the analysis of biopsy images under a microscope, determination of tumor type, selection of appropriate treatment methods, and identification of disease-related questions. According to the Ministry of Health's annual report in Gaza, there were 7,069 cases of breast cancer between 2009 and 2014, with 1,502 cases reported in 2014. In an era dominated by visual (...) information, where 65% of the population are visual learners, the Knowledge-Based System serves as an effective tool to aid long-term information retention by utilizing visual and textual resources. The system features a user-friendly interface, enabling students to diagnose the disease and expand their understanding. Furthermore, it can be accessed via smartphones. The Knowledge-Based System comprises three primary components: Firstly, it provides specific patient-oriented questions to familiarize students with the questioning process. Secondly, it facilitates the analysis of biopsy sample images to determine their integrity and identify potential infections. Although digital breast tomosynthesis has higher sensitivity than digital mammography and at least as high specificity, digital mammography remains the most common method for conducting mammographic screening. At the same time, mammography systems are now delivered “DBT-ready” and can be used for either digital mammography or digital breast tomosynthesis. In this paper, we ask whether it is ethically permissible to use such equipment for digital mammography, given its lower sensitivity. We argue it is not, and that clinics are (...) ethically required to use their DBTready equipment to screen with digital breast tomosynthesis whenever this is practically possible. Our argument relies on a comparison between digital breast tomosynthesis and a hypothesized improvement in the image quality of digital mammography. (shrink)

An increasing amount of twenty-first century metaphysics is couched in explicitly hyperintensional terms. A prerequisite of hyperintensional metaphysics is that reality itself be hyperintensional: at the metaphysical level, propositions, properties, operators, and other elements of the type hierarchy, must be more fine-grained than functions from possible worlds to extensions. In this paper I develop, in the setting of type theory, a general framework for reasoning about the granularity of propositions and properties. The theory takes as primitive the notion of a (...) substitution on a proposition (property, etc.) and, among other things, uses this idea to elucidate a number of theoretically important distinctions. A class of structures are identified which can be used to model a wide range of positions about the granularity of reality; certain of these structures are seen to receive a natural treatment in the category of M-sets. Breast cancer remains a significant health concern worldwide, necessitating the development of effective diagnostic tools. In this study, we employ a neural network-based approach to analyze the Wisconsin Breast Cancer dataset, sourced from Kaggle, comprising 570 samples and 30 features. Our proposed model features six layers (1 input, 1 hidden, 1 output), and through rigorous training and validation, we achieve a remarkable accuracy rate of 99.57% and an average error of 0.000170 as shown in the image below. Furthermore, (...) our investigation identifies the most influential features in breast cancer diagnosis, shedding light on the key determinants of malignancy. Notably, we find that factors such as fractal dimension_se, symmetry worst, compactness_worst, symmetry_se, and smoothness_se play pivotal roles in distinguishing between benign and malignant cases. This research contributes to the ongoing efforts to enhance breast cancer diagnosis, providing valuable insights into feature importance and showcasing the potential of neural networks in medical applications. Our findings have implications for improving early detection and treatment strategies, ultimately contributing to improved patient outcomes. (shrink)

Sensory substitution devices make use of one substituting modality to get access to environmental information normally accessed through another modality . Based on behavioural and neuroimaging data, some authors have claimed that using a vision-substituting device results in visual perception. Reviewing these data, we contend that this claim is untenable. When a user integrates a sensory substitution device into her life, the process involves perceptual learning, that is, ‘relatively long-lasting changes to an organism’s perceptual system that improve its ability to respond to its environment’ (Goldstone 1998: 585). In this paper, I explore ways in which the extensive literature on perceptual learning can be applied to help improve sensory substitution devices. I then use these findings to answer a philosophical question. Much of the philosophical debate surrounding sensory substitution devices concerns (...) what happens after perceptual learning occurs. In particular, should the resultant perceptual experience be classified in the substituted modality (as vision), in the substituting modality (as auditory or tactile), or in a new sense modality? I propose a novel empirical test to help resolve this philosophical debate. (shrink)

It is hoped that modern sensory substitution and augmentation devices will be able to replace or expand our senses. But to what extent has this been achieved to date? To what extent are the experiences created by sensory substitution devices like the sensory experiences that we are trying to replace? To what extent can we augment people’s senses providing them with new information and new experiences? The first aim of this introduction is to delve deeply into this question to discover (...) the usefulness of these devices, to outline the different sorts of experience that might be created, and what the evidence tells us about these experiences. While there are some reasons to be hopeful about the powers of sensory substitution devices, there are also reasons to wonder whether they will ever really have the practical applications that we hope they might have. The second aim is to look to see whether the study of modern sensory substitution and augmentation devices can shed light on the nature of our senses and perception in general. Much of the philosophical, psychological, and neuroscientific work that takes place concerning sensory substitution and augmentation is keenly aware of the possibility that it might help our understanding and it seeks to comprehend many different aspects of perception. In the substitutional framework, validity is truth under all substitutions of the nonlogical vocabulary. I develop a theory where □ is interpreted as substitutional validity. I show how to prove soundness and completeness for common modal calculi using this definition.

One of the central limitations of sensory substitution devices (SSDs) is their inability to reproduce the non-sensory feelings that are normally associated with visual experiences, especially hedonic and aesthetic responses. This limitation is sometimes reported to cause SSD users frustration. To make matters worse, it is unclear that improvements in acuity, bandwidth, or training will resolve the issue. Yet, if SSDs are to actually reproduce visual experience in its fullness, it seems that the reproduction of non-sensory feelings will be of (...) some importance. We offer a novel solution. Researchers can produce hedonic and aesthetic responses by eliciting these feelings artificially, pairing distal objects that should be pleasurable to pleasurable stimulus outputs from the SSD. We outline two strategies for accomplishing this: first, by means of a prefixed, hardwired, association of pleasant distal objects to pleasant stimulus outputs from the SSD; second, by means of a flexible, feedback-based association which creates associations based on a subject-directed matching of distal objects to patterns of stimuli from the SSD which the subject takes to have the corresponding hedonic properties. We evaluate some problems with both strategies, and we argue that the feedback-based strategy is more promising. Researchers can use this strategy to help the blind, allowing them to take pleasure in the objects they perceive using SSDs. (shrink)

The aim of this article is to provide a plausible conceptual model of a specific use of images described as substitution in recent art-historical literature. I bring to light the largely implicit shared commitments of the art historians’ discussion of substitution, each working as they do in a different idiom, and I draw consequences from these commitments for the concept of substitution by image—the major being the distinction between nonportraying substitution and substitution by portrayal. I then develop an argument that (...) substitution by image in the desired, nonportraying sense needs to be thought of in terms of a figurative representation of an image’s subject as a generic object, what I will call its figurative instantiation. This report highlights and explores five questions that arose from the workshop on sensory substitution and augmentation at the British Academy, March 26th through 28th, 2013.

It is widely held that counterfactuals, unlike attitude ascriptions, preserve the referential transparency of their constituents, i.e., that counterfactuals validate the substitution of identicals when their constituents do. The only putative counterexamples in the literature come from counterpossibles, i.e., counterfactuals with impossible antecedents. Advocates of counterpossibilism, i.e., the view that counterpossibles are not all vacuous, argue that counterpossibles can generate referential opacity. But in order to explain why most substitution inferences into counterfactuals seem valid, counterpossibilists also often maintain that counterfactuals (...) with possible antecedents are transparency‐preserving. I argue that if counterpossibles can generate opacity, then so can ordinary counterfactuals with possible antecedents. Utilizing an analogy between counterfactuals and attitude ascriptions, I provide a counterpossibilist‐friendly explanation for the apparent validity of substitution inferences into counterfactuals. I conclude by suggesting that the debate over counterpossibles is closely tied to questions concerning the extent to which counterfactuals are more like attitude ascriptions and epistemic operators than previously recognized. The 'substitution argument' purports to demonstrate the falsity of Russellian accounts of belief-ascription by observing that, e.g., these two sentences: (LC) Lois believes that Clark can fly. (LS) Lois believes that Superman can fly. could have different truth-values. But what is the basis for that claim? It seems widely to be supposed, especially by Russellians, that it is simply an 'intuition', one that could then be 'explained away'. And this supposition plays an especially important role in Jennifer Saul's defense of (...) Russellianism, based upon the existence of an allegedly similar contrast between these two sentences: (PC) Superman is more popular than Clark. (PS) Superman is more popular than Superman. The latter contrast looks pragmatic. But then, Saul asks, why shouldn't we then say the same about the former? The answer to this question is that the two cases simply are not similar. In the case of (PC) and (PS), we have only the facts that these strike us differently, and that people will sometimes say things like (PC), whereas they will never say things like (PS). By contrast, there is an argument to be given that (LS) can be true even if (LC) is false, and this argument does not appeal to anyone's 'intuitions'. The main goal of the paper is to present such a version of the substitution argument, building upon the treatment of the Fregan argument against Russellian accounts of belief itself in "Solving Frege's Puzzle". A subsidiary goal is to contribute to the growing literature arguing that 'intuitions' simply do not play the sort of role in philosophical inquiry that so-called 'experimental philosophers' have supposed they do. In their Anachronic Renaissance, Alexander Nagel and Christopher Wood identify two principles upon which, in fifteenth-century Europe, a work of art might establish its validity or authority: substitution and performance. It has become established wisdom that the dual schema of substitution and performance follows Hans Belting's dualism of the medieval cult of the image and the modern aesthetic system of art. This, I submit, is not just a mistake, but also prevents from evaluating one of the book's most ambitious contributions (...) to art-historical theory on its own merits. An analysis of the structure of the claims made by Nagel and Wood brings to light that the two concepts—substitution and performance—do not play the same role as the conceptual pair of Bild and Kunst in Belting's influential work. Goodman and Lederman (2020) argue that the traditional Fregean strategy for preserving the validity of Leibniz’s Law of substitution fails when confronted with apparent counterexamples involving proper names embedded under propositional attitude verbs. We argue, on the contrary, that the Fregean strategy succeeds and that Goodman and Lederman’s argument misfires.

In liberal moral theory, interfering with someone’s deliberate engagement in a self-harming practice in order to promote their own good is often considered wrongfully paternalistic. But what if self-harming decisions are the product of an oppressive social context that imposes harmful norms on certain individuals, such as, arguably, in the case of cosmetic breast surgery? Clare Chambers suggests that such scenarios can mandate state interference in the form of prohibition. I argue that, unlike conventional measures, Chambers’ proposal recognises that (...) harmful, discriminatory norms entail a twofold collective moral obligation: to eliminate the harmful norm in the long run, but also to address unjust harm that is inflicted in the meantime. I show that these two obligations tend to pull in opposite directions, thus generating a serious tension in Chambers’ proposal which eventually leads to an undue compromising of the second obligation in favour of the first. Based on this discussion, I develop an alternative proposal which, instead of prohibiting breast implant surgery, offers compensation for the disadvantages suffered by individuals who decide not to have surgery. The paper argues that philosophers commonly misidentify the substitutivity principle involved in Russell’s puzzle about substitutivity in “On Denoting”. This matters because when that principle is properly identified the puzzle becomes considerably sharper and more interesting than it is often taken to be. This article describes both the puzzle itself and Russell's solution to it, which involves resources beyond the theory of descriptions. There is widespread agreement, even among those who accept the possibility of backward causation, that it is impossible to change the past. I argue that this agreement corresponds to a relatively uninteresting understanding of what changing the past amounts to. In one sense it is indeed impossible to change the past: in no possible world is an action performed which makes the past in that world different from the past in that world. In another sense, however, it may be possible (...) to change the past: maybe in some possible world an action is performed which makes the past in that world different from the actual past. I argue that those who accept the possibility of backward causation are committed to accepting the possibility that the past changes in the latter sense. (shrink)

Expressivism is a blossoming meta-semantic framework sometimes relying on what Carter and Chrisman call “the core expressivist maneuver.” That is, instead of asking about the nature of a certain kind of value, we should be asking about the nature of the value judgment in question. According to expressivists, this question substitution opens theoretical space for the elegant, economical, and explanatorily powerful expressivist treatment of the relevant domain. I argue, however, that experimental work in cognitive psychology can shed light on how (...) the core expressivist maneuver operates at the cognitive level and that this: raises worries about the aptness of the expressivist question substitution and supports an evolutionary debunking argument against expressivism. Since evolutionary debunking arguments are usually run in favor of expressivism, this creates an obvious puzzle for expressivists. I wrap up by briefly responding to the objection that the debunking argument against expressivism overgeneralizes and, therefore, should be rejected. (shrink)

In perceptual experience we are directed towards objects in a way which establishes a real relation between a mental act and its target. In reading works of fiction we enjoy experiences which manifest certain internal similarities to such relational acts, but which lack objects. The substitution theory of art attempts to provide a reason why we seek out such experiences and the artifacts which they generate. Briefly, we seek out works of art because we enjoy the physiology and the phenomenology (...) of, for example, the experience of love or mountain climbing, and works of art serve as props for the promotion of substitutes for the corresponding genuine feelings. This paper proposes substitutional definitions of logical truth and consequence in terms of relative interpretations that are extensionally equivalent to the model-theoretic definitions for any relational first-order language. Our philosophical motivation to consider substitutional definitions is based on the hope to simplify the meta-theory of logical consequence. We discuss to what extent our definitions can contribute to that.

Is the brain the biological substrate of consciousness? Most naturalistic philosophers of mind have supposed that the answer must obviously be «yes » to this question. However, a growing number of philosophers working in 4e (embodied, embedded, extended, enactive) cognitive science have begun to challenge this assumption, arguing instead that consciousness supervenes on the whole embodied animal in dynamic interaction with the environment. We call views that share this claim dynamic sensorimotor theories of consciousness (DSM). Clark (2009) a founder and (...) leading proponent of the hypothesis of the extended mind, demurs, arguing that as matter of fact the biology of consciousness doesn’t allow for a brain, body and world boundary crossing architecture. We begin by looking at one of the arguments for DSM, the variable neural correlates argument. We then outline two criticisms that Clark has made of this argument and endorse his criticisms. However we finish up by using the case of sensory substitution to argue that something of this argument for DSM nevertheless survives. We suggest that Clark ought to concede sensory substitution as a case in which the conscious mind extends. (shrink)

A new reprogenetic technology, mitochondrial replacement, is making its appearance and, unsurprisingly given its promise to wash off our earthly stains --or at least the scourges of sexual reproduction--, John Harris finds only reasons to celebrate this new scientific feat.1 In fact, he finds mitochondrial replacement techniques (MRTs) so “unreservedly welcome” that he believes those who reject them suffer from “a large degree of desperation and not a little callousness.”2 Believing myself to be neither desperate nor callous, but finding myself (...) also no closer at all – not even after reading his article—to following Harris in welcoming these technologies wholeheartedly, it seems appropriate to respond. How do philosophers and psychologists receive paradigmatic cases from pathology? More specifically, how are some essential features of ‘normal’ cognitive, affective or perceptual functions derived from these pathological cases? In this paper, I argue that Maurice Merleau-Ponty offers a fecund answer to this question by putting forth a logic of supplementation in pathology that distinguishes the coping behavior of the organic world in contrast to an inorganic one. Supplementation, instead of substitution, marks the world of the living, particularly in its (...) higher forms, as it denotes a persistence through impairment governed by an organic norm. A prominent example of this appears in his reading of the patient Schneider case, a classic example from Goldstein and Gelb’s Gestalt psychology. While earlier commentators were interested in underlying whether or not Merleau-Ponty used this example to denote the persistence of a key function or the disruption of another, what has been missed, on my view, is a far more consequential point about pathologies and how they structure our relation to our world. -/- . We show that, contrary to conventional wisdom, Frege’s distinction between sense and reference does not reconcile a classical logic of identity with apparent counterexamples to it involving proper names embedded under propositional attitude verbs.

Sensory substitution devices (SSDs) aim to compensate for the loss of a sensory modality, typically vision, by converting information from the lost modality into stimuli in a remaining modality. “The vOICe” is a visual-to-auditory SSD which encodes images taken by a camera worn by the user into “soundscapes” such that experienced users can extract information about their surroundings. Here we investigated how much detail was resolvable during the early induction stages by testing the acuity of blindfolded sighted, naïve vOICe users. (...) Initial performance was well above chance. Participants who took the test twice as a form of minimal training showed a marked improvement on the second test. Acuity was slightly but not significantly impaired when participants wore a camera and judged letter orientations “live”. A positive correlation was found between participants' musical training and their acuity. The relationship between auditory expertise via musical training and the lack of a relationship with visual imagery, suggests that early use of a SSD draws primarily on the mechanisms of the sensory modality being used rather than the one being substituted. If vision is lost, audition represents the sensory channel of highest bandwidth of those remaining. The level of acuity found here, and the fact it was achieved with very little experience in sensory substitution by naïve users is promising. (shrink)

Millianism is reasonable; that is, it is reasonable to think that all there is to the semantic value of a proper name is its referent. But Millianism appears to be undermined by the falsehood of Substitutivity, the principle that interchanging coreferential proper names in an intentional context cannot change the truth value of the resulting belief report. Mary might be perfectly rational in assenting to ‘Twain was a great writer’ as well as ‘Clemens was not a great writer’. Her confusion (...) does not seem to preclude her from assenting to those sentences in a normal, understanding manner. That is, Assent-for-Mary is true: Mary can knowingly assent to ‘Twain was a great writer’ and ‘Clemens was not a great writer’. By Disquotation—the rough principle that if in ordinary circumstances one assents to “P”, then one believes that P—Mary believes that Twain was a great writer and she believes that it’s not the case that Clemens was a great writer. If Substitutivity were true, then since ‘Mary believes that Twain was a great writer’ is true, ‘Mary believes that Clemens was a great writer’ would have to be true too. But then Mary would amount to a refutation of the plausible principle Consistency that, roughly put, no rational adult can have occurrently held and reflectively considered and compared contradictory beliefs. Since Disquotation, Assent-for-Mary, and Consistency are true, Substitutivity has to go. (shrink)

Prior’s problem consists in the impossibility of replacing clausal complements of most attitude verbs by ‘ordinary’ NPs; only ‘special quantifiers’ that is, quantifiers like 'something' permit a replacement, preserving grammaticality or the same reading of the verb: (1) a. John claims that he won. b. ??? John claims a proposition / some thing. c. John claims something. In my 2013 book Abstract Objects and the Semantics of Natural Language, I have shown how this generalizes to nonreferential complements of various other (...) intensional predicates and argued for a Nominalization Theory of special quantifiers. In this paper, I will review and extend the range of linguistic generalizations that motivate the Nominalization Theory and show that they pose serious problems for higher-order and substitutional analyses of special quantifiers. Abstract—Breast cancer is considered the second most common cancer in women compared to all other cancers. It is fatal in less than half of all cases and is the main cause of mortality in women. It accounts for 16% of all cancer mortalities worldwide. Early diagnosis of breast cancer increases the chance of recovery. Data mining techniques can be utilized in the early diagnosis of breast cancer. In this paper, an academic experimental breast cancer dataset is (...) used to perform a data mining practical experiment using the Waikato Environment for Knowledge Analysis (WEKA) tool. The WEKA Java application represents a rich resource for conducting performance metrics during the execution of experiments. Pre-processing and feature extraction are used to optimize the data. The classification process used in this study was summarized through thirteen experiments. Additionally, 10 experiments using various different classification algorithms were conducted. The introduced algorithms were: Naïve Bayes, Logistic Regression, Lazy IBK (Instance-Bases learning with parameter K), Lazy Kstar, Lazy Locally Weighted Learner, Rules ZeroR, Decision Stump, Decision Trees J48, Random Forest and Random Trees. The process of producing a predictive model was automated with the use of classification accuracy. Further, several experiments on classification of Wisconsin Diagnostic Breast Cancer and Wisconsin Breast Cancer, were conducted to compare the success rates of the different methods. Results conclude that Lazy IBK classifier k-NN can achieve 98% accuracy among other classifiers. The main advantages of the study were the compactness of using 13 different data mining models and 10 different performance measurements, and plotting figures of classifications errors. (shrink)

It is sometimes said that simulation can serve as epistemic substitute for experimentation. Such a claim might be suggested by the fast-spreading use of computer simulation to investigate phenomena not accessible to experimentation (in astrophysics, ecology, economics, climatology, etc.). But what does that mean? The paper starts with a clarification of the terms of the issue and then focuses on two powerful arguments for the view that simulation and experimentation are ‘epistemically on a par’. One is based on the claim (...) that, in experimentation, no less than in simulation, it is not the system under study that is manipulated but a system that ‘stands-in’ for it. The other one highlights the pervasive use of models in experimentation. It will be argued that these arguments, as compelling as they might seem, are each based on a mistaken interpretation of experimentation and that, far from simulation and experimentation being epistemically on a par, they do not have the same epistemic function, do not produce the same kind of epistemic results. Objective: The use of neoadjuvant chemotherapy (NACT) in breast cancer is increasing. The management of locally advanced breast cancer differs due to the approach of the center to which the patient applied and the approach of the following physician. From this point of view, we aimed to evaluate the real life data of our region. -/- Methods: The study included 106 patients treated with neoadjuvant chemotherapy in the medical oncology clinic of two different university hospitals. Association between clinicopathological (...) features and pathological complete response (pCR) were analyzed. -/- Results: The pCR rate was higher in patients with negative hormone receptors and this difference was statistically significant (p:0.000). The rate of obtaining pCR increased as the NACT duration increased, and this increase was statistically significant. The mean NACT duration applied to the patients with pCR was 5.48 ± 0.22 months, and the mean NACT duration for those who could not obtain pCR was 5.01 ± 0.1 months (p:0.041). The recurrence rate of patients with pCR was 11.1%, while the recurrence rate of patients who could not obtain pCR was 31.6% (p:0.04). -/- Conclusions: Pathological response to chemotherapy is an important factor in determining prognosis. There appears to be a need for new biomarkers that allow the prediction of pCR and long-term outcomes. (shrink)

Throughout the Yemeni Civil War, western states have supplied weapons used in the indiscriminate bombing campaign conducted by the Saudis. In defence of their actions, British politicians have argued that they are exchanging weapons for influence, and using the influence obtained to encourage compliance with humanitarian law. An additional premise in the argument is that Britain is using its influence more benignly than alternative suppliers would use theirs if Britain were not on the scene. The idea is that Britain is (...) substituting itself for other, less scrupulous, interveners. I argue that, regardless of whether British substitution intervention could be justified in this way…, it is not in fact justified, because Britain has not plausibly used its influence to secure an amount of good sufficient to offset the various harms that its actions have created (or to discharge the expanded duties of rescue that greater influence entails). In addition, the article identifies the various forms that substitution intervention can take, and shows how the concept reveals hitherto neglected reasons to both support and oppose intervention in foreign conflicts. The concept of ecosystem services is a way of visualizing the instrumental value that nature has for human beings. Most ecosystem services can be performed by more than one species. This fact is sometimes used as an argument against the preservation of species. However, even though substitutability does detract from the instrumental value of a species, it also adds option value to it. The option value cannot make a substitutable species as instrumentally valuable as a non-substitutable species, but in many (...) cases, it can add enough value to make the species more valuable than the projects that threaten its existence. Block (Trends Cogn Sci 7:285–286, 2003) and Prinz (PSYCHE 12:1–19, 2006) have defended the idea that SSD perception remains in the substituting modality (auditory or tactile). Hurley and Noë (Biol Philos 18:131–168, 2003) instead argued that after substantial training with the device, the perceptual experience that the SSD user enjoys undergoes a change, switching from tactile/auditory to visual. This debate has unfolded in something like a stalemate where, I will argue, it has become difficult to determine whether the perception acquired (...) through the coupling with an SSD remains in the substituting or the substituted modality. Within this puzzling deadlock two new approaches have been recently suggested. Ward and Meijer (Conscious Cogn 19:492–500, 2010) describe SSD perception as visual-like but characterize it as a kind of artificially induced synaesthesia. Auvray et al. (Perception 36:416–430, 2007) and Auvray and Myin (Cogn Sci 33:1036–1058, 2009) suggest that SSDs let their users experience a new kind of perception. Deroy and Auvray (forthcoming) refine this position, and argue that this new kind of perception depends on pre-existing senses without entirely aligning with any of them. So, they have talked about perceptual experience in SSDs as going "beyond vision". In a similar vein, MacPherson (Oxford University Press, New York, 2011a) claims that “if the subjects (SSD users) have experiences with both vision-like and touch-like representational characteristics then perhaps they have a sense that ordinary humans do not” (MacPherson in Oxford University Press, New York, 2011a, p. 139). Although our concepts of “Mama,” “milk,” and “mice” have much in common, the suggestion that they are identical in structure in the mind of the prelinguistic child is mistaken. Even infants think about objects as different from substances and appreciate the distinction between kinds (e.g., mice) and individuals (e.g., Mama). Such cognitive capacities exist in other animals as well, and have important adaptive consequences.

Cortical plasticity is often invoked to explain changes in the quality or location of experience observed in rewired animals, in sensory substitution, in extension of the body through tool use, and in the rubber hand illusion. However this appeal to cortical plasticity may be misleading, because it suggest that the cortical areas that are plastic are themselves the loci of generation of experience. This would be an error, I claim, since cortical areas do not generate experience. Cortical areas participate in (...) enabling the interaction of an agent with its environment, and the quality of this interaction constitutes the quality of experience. Thus it is not plasticity in itself, but the change in modes of interaction which plasticity allows, which gives rise to the change of experience observed in these studies. (shrink)
